单侧椎体成形术配合补肾强筋胶囊治疗老年胸腰椎压缩性骨折临床研究

摘　　要：目的:探讨单侧椎体成形术配合补肾强筋胶囊治疗老年胸腰椎压缩性骨折的临床疗效。方法:将70例患者按随机数字表发分为观察组、对照一组和对照二组共三组。对照一组18例,采用基础治疗(常规抗骨质疏松和抗炎治疗)和经皮单侧椎体成形术治疗;观察组38例,在对照一组治疗基础上术后给予补肾强筋胶囊(杜仲、血竭、三七、骨碎补等),术后第2天开始腰背肌功能锻炼,并戴腰围保护开始下地行走;对照二组14例,采用基础治疗及卧床垫枕治疗。对患者治疗前、术后即刻、术后1周、术后1个月、术后6个月、术后12个月进行视觉模拟疼痛评分,同时通过侧位X线片测量病椎前缘高度和Cobb角变化及12个月内腰背痛复发次数。结果:VAS评分观察组、对照一组,治疗后即刻、术后1周、术后1个月、6个月、12个月与治疗前比较,明显降低(P<0.05);对照二组治疗后即刻、术后1周、术后1月与治疗前比较,降低不明显(P>0.05)、治疗后6个月、12个月与治疗前比较,明显降低(P<0.05);三组治疗后6个月、12个月,VAS评分无统计学意义(P>0.05);椎体高度恢复三组之间无统计学意义。三组治疗后Cobb角比治疗前减少;观察组、对照一组治疗后1个月、6个月、12个月与治疗后1周比较,未见Cobb角明显丢失;再发腰背痛人次观察组、对照一组明显减少,对照二组增多,差异具有统计学意义(P<0.05)。结论:椎体成形术配合补肾强筋胶囊治疗老年性胸腰椎压缩性骨折可缓解疼痛,维持病椎高度及脊柱的稳定性,减少再发腰背痛发生人次。Objective ：To investigate the clinical curative effect of unilateral vertebral plasty with Bushen Qiangjin Capsule in the treat- ment of elderly thoracolumbar vertebral compression fractures. Methods：70 cases of patients were randomly divided into the three groups, namely, the observation group, the two control groups. The first control group of 18 cases was given foundation treatment （ conven- tional anti osteoporosis and anti-inflammatory therapy） and percutaneous unilateral vertebral plasty treatment;The observation group of 38 cases was treated with Bushen Qiangjin Capsule（ eucommia, dragon＇s blood, notoginseng, rhizoma drynariae, etc. ） on the basis of the first control group, and lumbar back muscle functional exercise was taken on the second day post operative, and started to walk round under wearing waist protection. The second control group of 14 cases was given basic treatment and bed and pillow therapy. Patients were taken visual analog pain score before treatment,immediately after operation, 1 week, 1 month,6 months and 12 months post opera- tion, and at the same time, the change of week vertebral front height and Cobb angle were measured through the lateral x-ray and recur- rence frequency of lumbago and backache within 12 months were calculated. Results：VAS score in the observation group immediately after operation, 1 week, 1 month,6 months and 12 months post operation were all significantly decreased compared with before treatment （ P 〈 0.05 ）, and the same situation in the first control group. VAS score in the second group immediately after operation, 1 week and 1 month post operation all were not obvious decreased compared with before treatment （ P 〉 0.05 ）, and 6 months and 12 months after treatment ~AS score were significantly decreased compared with before treatment （ P 〈 0.05 ） ; 6 months and 12 months of the three groups after treatment, VAS score had no statistical significance （P 〉 0.05 ） ;Vertebral height restoration had no statistical significance o
