摘  要:目的 比较慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者最大摄氧量(maximum oxygen consumption,VO2 max)与BODE指数关系,进一步评价BODE指数客观性.方法 27例稳定期COPD(轻到极重度)患者进行心肺运动实验(cardiopulmonary exercise test,CPET)测定、6分钟步行实验(6-minute walk test,6MWT)与改良的医学研究委员会呼吸困难评分(the modified Medical Research Council dyspnea scale,MMRC)、静态肺功能测定,计算体质量指数.比较BODE指数值及最大每公斤体质量摄氧量(VO2 max/kg)替代6MWT的改良BODE指数值.结果 在COPD患者中VO2 max/kg与BODE指数呈负相关(r=-0.515,P<0.01);用VO2 max/kg替代6MWT的改良BODE指数与包括6MWT的BODE指数之间为正相关(r=0.955,P=0.000).结论 VO2 max作为运动功能评定金标准,它与BODE指数有较强相关性,同时VO2 max/kg替代6MWT后改良BODE指数与原始BODE指数相关性均较好,进一步证实BODE指数的可靠性.Objective To compare the relationship between maximum oxygen consumption (VO2max) in patients with chronic obstructive pulmonary disease (COPD) and the BODE index and value the objective of the BODE index.Methods Twenty-seven patients with mild-to-most severe COPD performed cardiopulmonary exercise test (CPET),6-minute walk test (6MWT),the modified Medical Research Council dyspnea scale (MMRC),pulmonary function,body mass index (BMI).The BODE index and the modified BODE index with VO2 max/kg (maximum oxygen consumption/kg,VO2 max/kg)replacing the 6MWT were compared.Results The VO2 max/kg was negatively correlated with the BODE index in patients with COPD (r =-0.515,P <0.01).Using VO2max/kg to replace the 6MWT in BODE index was positively correlated with the BODE index with 6MWT (r =0.955,P =0.000).Conclusions The VO2max as the primary standard for exercise evaluation,has a significant correlation with the BODE index.The modified BODE index using VO2 max/kg instead of 6MWT and the original BODE index have a similar result.
