摘　　要：目的:探讨自助垂直牵引整复治疗单纯胸腰椎压缩性骨折的优越性。方法:选取2011年1月一2012年12月单纯胸腰椎骨折住院病人100例,随机分为治疗组与对照组,治疗组采用自助垂直牵引整复治疗,对照组采用垫枕复位治疗。观察两组治疗前后疼痛评分、伤椎Cobb角与伤椎高度的变化及疗效,并于治疗结束后1个月、3个月随访临床疗效及SF-36评分的变化。结果:两组治疗前疼痛定量评分、伤椎Cobb角与伤椎高度间差异无统计学意义,治疗结束时、随访1个月时及随访3个月时临床疗效差异具有统计学意义,且治疗组SF-36评分在生理机能、生理职能、躯体疼痛、精神健康维度改善上优于对照组,差异有统计学意义。结论:自助式垂直牵引整复治疗单纯胸腰椎骨折,通过主动与被动功能锻炼相结合的练功方法,将整复与锻炼有机的早期结合在一起,有助于提高单纯胸腰椎骨折患者的生活质量及促进患者的功能恢复,与传统垫枕复位方法相比有明显优势。Objective： To investigate the superiority of self-help vertical traction and restitution to treat simple thoracolumbar compression fracture. Method： 100 simple thoracolumbar compression fracture patients from the inpatient department since January 2011 to December 2012 were randomly divided into therapy and control groups.The therapy group was treated by self-help vertical traction and restitution, and the control group was treated by padding the pillow under the back.Observations on the change of the VAS score, Cobb angle and height of the fractured vertebra and the therapeutic effect, and the therapeutic effect were carried out.SF-36 Health Survey Questionnair was made between the two groups at the end of the treatment, 1 and 3 months after the treatment. Result ： The pain quantitation score, Cobb angle and height of the fractured vertebra had no difference between the two groups before the treatment, but had difference after the treatment and 1 and 3 months follow-up, while the SF-36 Health Survey Questionnair was also different in PF, RP, BP and RE. Conclusion ： The self-help vertical traction and restitution combined with the initiative and passive train methods is much more useful for treatment and rehabilitation of the thoracolumhar compression fracture.
