临床资质对安顺市的疾控与医疗机构结核病防治的影响

摘　　要：目的分析临床资质对安顺地区疾控与医院结核病防治工作的影响情况,为结核病防治决策提供科学依据。方法在市卫生局的领导下对2011年安顺市及各县区医疗机构与疾控机构进行检查、督导,对相关病例进行访视调查,对比分析疾控中心与医院结核病诊断、治疗、管理等的不同,了解临床资质对疾控结防机构结核病防治方面的影响,并寻找解决途径。结果因缺乏临床医疗资质,安顺市各县疾控结防机构与医疗机构在结防人员专业构成上差异有统计学意义(χ2=53.17,P<0.01)、治疗肺结核病人的方法上差异有统计学意义(χ2=168.39,P<0.01)、强化期病例在治疗期间感觉上差异有统计学意义(χ2=8.36,P<0.01)、对医生信任度差异也有统计学意义(χ2=4.98,0.01<P<0.05)。结论临床医疗资质缺乏导致疾控机构在肺结核病诊断、治疗等方面存在一定的困难和障碍,是影响结核病防治的关键因素之一,结核病防治模式存在进一步完善的可能。Objective To analyze the influence of clinical qualification on prevention and control of tuberculosis in center for disease control and prevention（CDC） and hospital of Anshun.Methods Medical institutions and CDC were inspected and supervised.The related cases were investigated and the difference of diagnosis,therapy and management of tuberculosis medication were compared and analyzed.At the same time,we need to learn about the influence of clinical qualification on prevention and control of tuberculosis and seek the corresponding solutions.Results Because of the lack of clinical medical qualification,there were significant differences in professional composition（χ2= 53.17,P 0.01）,treatment methods（χ2= 168.39,P 0.01）,the patients felling in the treatment period（χ2= 8.36,P 0.01）,and the trust degree about doctor（χ2= 4.98,0.01 P 0.05）.Conclusions The lack of clinical qualification cause some problems of tuberculosis diagnosis and medication therapy,which is the key factors of TB prevention and control.There exists possibility to further improve the TB control mode.
