注射型人工骨结合伤椎置钉治疗胸腰椎骨折

摘　　要：目的探讨可注射型人工骨结合包括伤椎的椎弓根钉系统治疗胸腰椎骨折的临床疗效。方法 2011年1月至2013年5月,对21例简单胸腰椎骨折患者,经肌间隙入路,行椎弓根螺钉固定伤椎及相邻上下椎体,并经伤椎椎弓根植入可注射型人工骨,术前、术后行X、CT检查并随访,评估椎体高度、Cobb角恢复和维持情况,观察人工骨的吸收情况、组织相容性。结果平均随访11.4个月,伤椎平均高度、Cobb角恢复、维持良好。所有病例无长期发热、渗出现象,人工骨均在3月内吸收。结论可注射型人工骨结合伤椎置钉治疗胸腰椎骨折有效恢复、维持了椎体高度,伤椎植入可注射型人工骨是安全、有效的。Objective To observe the clinical effect of treatment of thoracolumbar fracture with injectable bone substitute and pedicle screw fixation including the injured vertebra.Methods From January 2011to May 2013,21 cases with thoracolumbar fracture were recruited,grafed with injectable bone substitute（IBS）,fixed with pedicle screws including the injured vertebra via paraspinal approach.Radiographic and CT examination were performed before and after surgery,and the patients were followed up postoperation.Several parameters were detected for value： height of fractured vertebra,the Cobb ＇s angle on sagittal plane,restoration of IBS,biocompatibility of the IBS to host.Results The patients were followed up for average 11.4 months.The height of the fracture vertebra and the Cobb＇s angle were restored and maintained well.Among these cases,long-term fever and exudation were not found.All IBS were absorbed within 3 month.Conclusions Treating thoracolumbar vertebral fracture this way restore and maintain the height of fractured vertebra and the Cobb＇ s angle well,and filling the injured vertebra with IBS is proved safely and effectively.
