改良早期预警评分系统在CCU危重症患者中的应用效果

摘　　要：目的 探讨改良早期预警评分系统在CCU患者中的应用效果.方法 选取2011年6-12月收住昆明医科大学第二附属医院心血管内科CCU的285例患者作为研究对象,按照随机数字法随机分为对照组142例及观察组143例.对照组采用传统的监测方案即根据医生医嘱进行常规监测,观察组根据改良早期预警评分（MEWS）分值进行不同级别的预警性护理干预.比较分析两组意外事件发生率、抢救成功率、患者满意度及平均住院日.结果 观察组患者意外事件发生率为5.59％,低于对照组的16.90％,差异有统计学意义（x2=9.139,P=0.013）;观察组患者满意度及抢救成功率分别为95.10％,75.00％,分别高于对照组的87.32％,29.20％,差异均有统计学意义（x2分别为5.390,6.016;P＜0.05）；观察组患者平均住院日为（4.8±2.2）d,少于对照组的（7.3±1.5）d,差异有统计学意义（t=5.40,P＜0.05）.结论 MEWS系统简便、实用,能及早发现和处置危重病例,减少医疗隐患,有利于在CCU护理工作中推广应用.Objective To discuss the effect of Modified Early Warning Score （MEWS） on CCU patients.Methods Totals of 285 CCU patients who were hospitalized between June and December in 2011 in Cardiovascular Department of the Second Affiliated Hospital of Kunming Medical University were chosen and randomly divided into the observation group （143 cases） and the control group （142 cases）.The control group received traditional monitoring scheme according to doctor＇ s advice,while the observation group used MEWS for warning nursing intervention of different levels.Incidence rate of accidents,successful rate of rescue,satisfaction degree of patients and average day of hospitalization were analyzed and compared between two groups.Results The incidence rate of accidents was 5.59％ in the observation group and 16.90％ in the control group,with statistically significant difference （x2 =9.139,P =0.013）.The satisfaction rate of patients and successful rate of rescue was respectively 95.10％ and 75.00％ in the observation group,both higher than 87.32％ and 29.20％ in the control group,and the differences were statistically significant （x2 =5.390,6.016,respectively; P ＜ 0.05）.The average day of hospitalization was （4.8 ± 2.2）d in the observation group,less than （7.3 ± 1.5） d in the control group,and the difference was statistically significant （t =5.40,P ＜0.05）.Conclusions The Modified Early Warning Score （MEWS） is simple and effective,which can detect and rescue critical cases early,reduce medical risk efficiently,and is compatible with extended applications in CCU nursing work.
