摘　　要：目的报道应用游离腓动脉穿支皮瓣修复手部皮肤软组织缺损的方法和临床效果。方法2009年2月-2013年1月，对12例手部不同部位、形态的皮肤软组织缺损采用游离腓动脉穿支皮瓣进行修复，切取皮瓣面积为4cm×6cm-8cm×13cm，以肌皮、肌间隔穿支为蒂。动脉血管蒂与受区血管采用端端或端侧吻合，供区创面直接缝合或游离植皮。结果本组12例皮瓣全部成活，术后随访3～12个月．1例指腹皮瓣因臃肿行皮瓣修整术后手指屈曲功能接近正常，余11例皮瓣的外观与功能恢复良好。结论应用腓动脉穿支皮瓣，可设计切取不同形状、大小的皮瓣，以适应并修复手部不同部位、形态的皮肤软组织缺损，均获得良好的修复效果。Objective To report the method and clinical effect of free peroneal artery perforators flap to repair skin and soft tissue defect of hand. Methods 12 cases with skin and soft tissue defects of hand were treated from February 2009 to January 2013 with free peroneal artery perforators flap to. The size of the flaps ranged from 4 cm×6 cm-8 cm×13 cm. In most cases the musculocutaneous branch or muscle interval branch was chosen as the vascular pedicle for anastomosis. The blood vessels were anastomosed in an endto-end or end-to-side fashion.The donor site was closed directly or flee skin graft processed. Results All the adipofascial flaps survived uneventfully. Follow-up time ranged from 3 to 12 months. Flap bulkiness was seen in 1 case which underwent debulking operation in 3 months later, and the flexion function of finger approached nornal. The appearance of other 11 cases was satisfactory with only slight flap bulkiness. Conclusion It can be designned for different shapes, different parts fit and the size of flap to repair hand skin and soft tissue defect by peroneal artery perforators flap, and good effects are obtained.
