腰硬联合麻醉与局麻下施实吻合器痔上黏膜环切术麻醉效果对照研究

摘　　要：摘要目的：探讨局麻与腰硬联合麻醉在环状脱垂痔患者中的麻醉效果和安全性。方法：运用随机数字表法将2010年5月~2012年5月在我院治疗行吻合器痔上黏膜环切术的154例环状脱垂痔患者分为A组和B组，A组患者在腰硬联合麻醉下行吻合器痔上黏膜环切术治疗，B组患者在局麻下行吻合器痔上黏膜环切术治疗，比较两组患者麻醉效果和安全性。结果：B组患者术后15分钟、1小时、2小时、4小时和8小时视觉模拟评分（VAS）明显低于A组患者，差异有统计学意义（P〈O．05），B组患者在恶心呕吐、皮肤瘙痒和尿潴留发生率明显低于A组，差异有统计学意义（P〈O．05），而且B组患者对手术满意度明显高于A组，差异有统计学意义（P〈O．05）。结论：局麻下行吻合器痔上黏膜环切术治疗环状脱垂痔麻醉效果好，具有安全高效、不良反应少和疼痛程度轻等特点。Objective： To investigate the anesthetic efficacy and safety of procedure for prolapse and hemorrhoids for patients with ring prolapse hemorrhoid by spinal-epidural anesthesia and local anesthesia. Methods： The clinical data of one hundred and fifl3,-four patients with ring prolapse hemorrhoid by procedure for prolapse and hemorrhoids who were treated in our hospital from May 2010 to May 2012 were collected, then patients were divided into A group and B group by using a random number table, patients in A group was given spinal-epidural anesthesia, but patients in B group was given local anesthesia, the other measures were the same, the anesthesia effect and safety of them was evaluated. Results： Visual analogue scale in patients in B group was lower than that in A group at the points of fifteen minutes, one hour, two hours, four hours and eight hours （P〈0.05）, the incidence of nausea and vomiting, skin pruritus and urinary retention in B group was lower than that in the control group （P〈0.05）, surgery satisfaction in B group was higher than that in the control group （P〈0.05）. Conclusions： The anesthesia effect of procedure for prolapse and hemorrhoids for patients with ring prolapse hemorrhoid local anesthesia is good and safe, with high efficiency, less adverse reactions and less pain.
