超声引导下经胸前区腋静脉置管术在神经外科手术中的应用

摘　　要：目的:探讨实时超声引导下腋静脉置管术在神经外科围手术期患者中的安全性及可行性。方法:对10例拟行后颅窝手术的神经外科患者行超声引导下的经胸腋静脉穿刺置管术。穿刺前采用超声图像对腋静脉第三段的横截面、纵切面进行位置确认,然后采用平面内技术行实时超声引导下腋静脉穿刺置管术。结果:10例患者均穿刺顺利并成功在腋静脉置入静脉导管,无血管损伤、气胸等并发症。结论:神经外科后颅窝手术患者采用实时超声引导下的腋静脉穿刺置管术安全、有效。Objective：To evaluate the safety and feasibility of axillary vein catheterization by real-time ultrasound-guided tech- nique in neurosurgical patients. Methods： Ultrasound-guided transthoracic axillary vein catheterization was carried out in 10 neu- rosurgical patients to undergo posterior fossa surgery. The anatomic structure was confirmed using transverse, longitudinal and Doppler flow images. The needle tip was advanced into the axillary vein under the guide of real-time ultrasound using an in-plane technique. Results： All the 10 patients completed successfully without vascular injury, pneumothorax and other complications. Conclusions： It＇s a safe and effective way of using real-time ultrasound-guided in-plane technique in axillary vein catheterization in posterior fossa surgeries.
