摘 要:目的:探讨临床上氧气湿化瓶科学、合理的更换频率,优化医疗资源,减少患者的医疗费用。方法:需要持续7 d吸氧的61例患者,在使用氧气湿化瓶的第1天至第7天的上午8∶00,分别对其使用的氧气湿化瓶瓶口及湿化液进行采样,并更换湿化液。由实验室取液划板,48 h统计结果,如有细菌生长,挑取菌落进行细菌计数和细菌鉴别。以不检出致病性微生物为合格。采用SPSS 14.0软件进行统计学分析,以第1天结果为对照组,以第2天至第7天结果为实验组,应用χ2检验对比实验组及对照组氧气湿化瓶的不合格数,以P<0.05为差异有统计学意义。结果:氧气湿化瓶使用的第2、3、4、5天,氧气湿化液和湿化瓶口细菌检出数与第1天比较,差异无统计学意义;第6天及第7天湿化液的细菌检出率分别为54.1%(33/61)和90.2%(55/61),均高于第1天[9.8%(6/61)],差异均有统计学意义(χ2=27.475和78.721,P<0.01);第6天及第7天氧气湿化瓶口细菌检出率分别为27.9%(17/61)和60.7%(37/61),均高于第1天[9.8%(6/61)],差异均有统计学意义(χ2=6.483和34.513,P<0.05和0.01)。结论:一次性氧气湿化瓶在使用第6天开始,无论是湿化液还是湿化瓶口,细菌检出均显著增加,差异有统计学意义,所以建议在使用氧气湿化瓶的第6天更换湿化瓶。Objective:To explore the scientific,reasonable changing frequency of oxygen humidifier bottles and to optimize the medical resource and reduce the medical cost of patients.Methods:Patients who need 7-day continuous oxygen therapy were selected.Samples of the oxygen humidifier bottlenecks and humidifier liquid at 8∶00 am daily from day 1 to day 7 were taken.And the humidifier liquid was changed.Samples were sent to the lab for bacterial culture.Crossed bacterial plate culture method was used to count the bacteria colony,and checked the result after 48 h.If bacteria grew,bacterial counting and identification of bacterial strain was continued.When there were no pathogenic microorganisms detected,the sample was qualified.SPSS 14.0 statistical software andχ2 test were used to analyze the difference among the bacterial counting result of humidifier liquid and the bottles between day 1(control group)and day 2 to day 7(experimental group).Results:The bacterial counting results of humidifier liquid or the bottlenecks on day 2,day 3,day 4 and day 5 had no significant difference compared with that on day 1.The bacterial counting results of humidifier liquid on day 6 and day 7 were 54.1 %(33/61)and 90.2 %(55/61),which was higher than that on day 1[9.8%(6/61)],the differences were statistically significant(χ2=27.475 and 78.721,P〈0.01).The bacterial counting results of the bottlenacks on day 6 and day 7 were 27.9 %(17/61)and 60.7 %(37/61),which was higher than that on day 1[9.8 %(6/61)],the differences were statistically significant(χ2=6.483 and 34.513,P〈0.05 and 0.01).Conclusions:Oxygen humidifier bottle should be changed after using for 6 days.
