静脉旋切治疗下肢大隐静脉曲张60例体会

摘　　要：目的：临床评价TIPP治疗下肢浅静脉曲张的疗效。方法：2009年3月-2011年12月在我院住院的118例大隐静脉曲张患者中，随机分为两组。对照组（传统手术组）58例采用传统手术，实验组（TIPP组）60例采用niVex System对60例大隐静脉曲张病人进行透照下动力静脉曲张旋切术。结果：TIPP组平均每条肢体手术时间（25．4±3．6）min，切口数量平均（4．5±0．5）个，出血量平均（32±6）ml。平均住院4．5d。对照组手术时间较实验组明显延长（P〈0．05），切口数量多于实验组（P〈0．05），术中出血量大于实验组（P〈0．05），平均住院12．8d。TIPP组总并发症率54．7％（47／86），对照组总并发症率65％（54／83），两组间上述各并发症发生率差异无统计学意义（X^2=1．9031，P=0．168〉0．05）。TIPP组术后复发率0。结论：TIPP与传统手术比较其优势明显，TIPP具有微创、手术时间短、术后无复发、且美容效果好的优点。Objective： Evaluation of TIPP clinical efficacy of treatment of lower extremity varicose veins. Method： In 2009, taking TriVex System in 60 patients with great saphenous vein in the patient through the op- eration according to peeling under the power varicose veins. Results： TIPP group average for each limb surgery （25.4 ±3.6） min, and cut the number of average （4.5 ± 0.5）, average blood loss（S2± 6） ml. Average length of stay of 4.5 days. Operation time of the control group than the experimental group was significantly prolonged （P 〈 0.05）, incision number more than the experimental group （P 〈 0.05）, blood loss greater than the experi- mental group（P〈0.05）, average length of stay 12.8 days. TIPP group, the total complication rate of 54.7% （47/86）, 65% of the total complication rate of the control group（54/S3）, the above complication rate was no significant difference between the two groups（X^2 = 1. 9031, P = 0. 168 〉 0.05）. The TIPP postoperative recur- rence rate of 0. Conclusion： TIPP has great advantages compared with traditional surgery significantly, TIPP has a minimally invasive, surgical time is short, with no postoperative recurrence, and the advantages of good cos- metic results.
