术前腹部加压卧床排尿训练对移植肾活检术后排尿方式的影响

摘　　要：目的探讨移植肾活检术前行腹部加压卧床排尿训练对术后排尿方式的影响。方法方便性抽样选择福州市某三级甲等医院2011年1-10月移植肾活检术患者45例,采用术前抽签法将其分为两组,对照组22例,观察组23例。对照组穿刺前1d进行常规卧床排尿训练;观察组在穿刺前1d在移植肾区放置0.5kg沙袋腹部包扎再进行卧床排尿训练5次以上,直至患者感到自然顺畅地排出尿液为止。比较两组患者的术后排尿方式(自行排尿、诱导排尿、导尿)。结果观察组23例患者中有20例自行排尿,3例诱导排尿,无一例需留置导尿;对照组22例患者中有13例自行排尿,5例诱导排尿,4例需留置导尿。观察组自行排尿率高于对照组(P<0.05)。结论对移植肾患者穿刺活检术前进行腹部加压卧床排尿训练,可以促进患者术后正常自行排尿,减少术后排尿困难、尿潴留的发生,增加患者的舒适度。Objective To explore the effects of preoperative abdominal pressure urination in bed training on urination mode for patients after renal allograft biopsy. Methods By convenience sampling,totally 45 patients with renal allograft biopsy were randomly divided into the control group （n=22） and observation- al group （n=23）. Patients in control group were given regular urination in bed training one day before bi- opsy, while patients in the observational group were given 0. 5kg sand bags placed in the transplanted kid- ney area and urination training in bed at least five times one day before biopsy until urine was discharged smoothly. Urination ways were compared between the two groups （natural emiction, revulsive emiction, urinary catheterization）. Results In the observational group,a total of 20 patients urinated by themselves, and 3 cases were induced to urinate, no one need catheterization . However, only 13 patients in control group urinate by themselves, 5 cases were induced voiding, and 4 patients required catheterization. Natural emiction rate of patients in observational group was higher than which in the control group （P〈0.05）. Conclusion The abdominal pressure training on bladder voiding before allograft biopsy can promote natural emiction, reduce urinary retention,and improve the comfortable feelings of the patients.
