腹腔镜手术与开腹手术治疗急性阑尾周围脓肿疗效比较

摘　　要：目的比较腹腔镜手术和开腹手术治疗急性阑尾炎周围脓肿的临床疗效。方法将120例急性阑尾炎周围脓肿患者随机分为观察组、对照组,每组60例。两组患者均给予全麻,采用头低足高位。观察组采用腹腔镜手术治疗,对照组采用传统开腹治疗。两组均于术后给予抗生素对症治疗5 d。观察两组手术持续时间、引流时间、切口长度、术后卧床时间、术后镇痛剂使用情况、肛门排气时间以及残端出血、切口感染、肠梗阻等并发症发生情况。结果两组患者均顺利完成手术,手术成功率达100.00%。下床时间、住院时间、术后镇痛药使用率、治疗总费用、肛门排气时间、体温恢复正常时间、白细胞恢复正常时间、并发症发生情况等术后相关指标两组相比差异有统计学意义(P﹤0.05)。结论与开腹手术相比,腹腔镜手术治疗急性阑尾炎,具有创伤小、术后恢复快、术后并发症少等优势。OBJECTIVE To compare the clinical efficacies of laparoscopic surgery and open surgery for acute appendicitis abscess. METHODS 120 patients with acute appendicitis abscess patients were randomly divided into observation group and the control group, 60 cases in each group. The patients in two groups were given general anesthesia, with Trendelenburg position. Laparoscopic surgery treatment was used in the observation group and the control group used conventional open treat- ment. Two groups were given antibiotics after symptomatic treatment for five days. Then observed operation time, drainage time, incision length, postoperative bed time, postoperative analgesic use, anal exhaust time and stump bleeding, wound infection, bowel obstruction and other complications. RESULTS Two groups were successfully completed surgery, the surgi- cal success rate was 100.0（0~. Bed time, hospital stay, postoperative analgesic usage, total treatment cost, anal exhaust time, the body temperature returned to be normal time, the white blood cells to restore be normal time, circumstances such as postoperative complications related indicators were compared in two groups, it found there were significant differences （P 〈 0.05）. CONCLUSION Compared with open surgery, laparoscopic surgery for acute appendicitis has advantages like small trauma, rapid postoperative recovery and fewer complications.
