两种鼻中隔矫正术式术后并发症的比较研究

摘　　要：目的比较鼻内镜下鼻中隔黏膜下切除术与鼻内镜下改良鼻中隔成形术两种术式的术后并发症。方法将陕西电子四○九医院2006年8月至2011年2月收治的鼻中隔偏曲患者363例依据随机数字表法双盲分为两组:鼻中隔黏膜下切除术组(176例)行鼻内镜下鼻中隔黏膜下切除术;改良鼻中隔成形术组(187例)行鼻内镜下改良鼻中隔成形术。术后随访24个月。比较两组术后并发征情况。结果鼻内镜下鼻中隔黏膜下切除术组术后发生鼻中隔穿孔8例,鼻中隔摆动23例,鼻梁塌陷7例,鼻小柱退缩5例;鼻内镜下改良鼻中隔成形术组除1例鼻中隔粉碎性骨折合并偏曲患者术后发生鼻中隔摆动、1例糖尿病患者因术后合并感染造成鼻中隔穿孔外,其他均无鼻梁塌陷、鼻小柱退缩、鼻中隔穿孔、鼻中隔摆动等术后并发症发生,两种术式术后并发症发生率比较差异有统计学意义(P<0.05)。结论鼻内镜下改良鼻中隔成形术较鼻内镜下鼻中隔黏膜下切除术术后并发症少,从现代微创及功能性手术角度来看优于后者。Objective To compare the postoperative complications of intranasal endoscopic resection of the nasal septum mueosa and the improved intranasal endoscopic septoplasty. Methods From Shaanxi Elec-tron 409 Hospital during Aug. 2006 and Feb. 2011,363 cases of patients with deviated nasal scptum were randomly divided into two groups：group A（ submucous resection of nasal scptum group, 176 cases） in treat- ment with intranasal endoscopic resection of the nasal septum mucosa; group B （modified nasal septum figura-tion group, 187 cases） with the improved intranasal endoscopic septoplasty. All patients were followed up for at least 24 months. The postoperative complications of the two groups were compared. Results In group A, postoperative perforation of nasal scptum was observed in 8 cases, nasal septum swing in 23 eases, bridge collapse in 7 cases, nasal columella retreat in 5 cases. In group B,except 1 case of nasal septum swing of na-sal septum comminuted fractures combined with nasal septum deviation combined with swing, 1 case of perfo-ration of nasal septum caused by postoperative infection in a diabetic patient, there were no postoperative complications such as bridge collapse, nasal columella retreat, nasal septum perforation, or nasal septum swing. There were significant differences between the two groups in the incidence of complications（ P 〈 0.05 ）. Conclusion The improved intranasal endoscopic septopiasty,with less postoperative complications,is better than the intranasal endoscopic resection of the nasal scptum mucosa from the perspectives of modem minimally-invasivcncss and functionality.
