摘　　要：目的探讨使用3 mm多功能针形器械隐瘢痕腹腔镜胆囊切除术的可行性。方法 12例确诊为无严重胆囊炎症的胆囊息肉或胆囊结石患者,使用我们研制的3 mm多功能针形器械完成隐瘢痕腹腔镜胆囊切除术。结果 12例手术均获成功,无中转常规腹腔镜手术病例,均未放置引流管。前6例手术平均耗时为26.2 min,后6例手术平均耗时为17.8 min。所有患者术后1 d即半流质饮食,第2天出院,出院半月时患者腹部无明显手术瘢痕。随访1个月所有患者均无出血或胆漏等并发症发生。结论使用3 mm多功能针形器械隐瘢痕腹腔镜胆囊切除术安全、可行,术后瘢痕隐蔽。在非急性胆囊炎手术中,该术式操作难度与常规腹腔镜手术相当,但较之单孔腹腔镜胆囊切除术难度降低,为腹腔镜手术的更微创化发展提供了另一可行途径。Objective To discuss the feasibility, methods and clinical application of a new scar-hidden laparoscopic cholecystectomy using 3ram mutifunctional needle-shaped instruments. Methods The clinical data of 12 patients with gallbladder diseases undergoing sear-hidden laparoscopic cholecystectomy was retrospectively analyzed. All operations were performed using 3mm mutifunctional needle-shaped instruments. Results All the operations were successfully performed. No drainage tube was placed and postoperative bleeding and biliary leakage did not occur. There was no visible scar on the abdominal wall. The mean operation time of the early six cases was 26.2min, the following six cases 17.8rain. Conclusions The new scar-hidden laparoscopic cholecystectomy is a safe and feasible operation. The scar was not obvious postoperation. Compared with Laparo-endoscopic single-site surgery, it was less difficulty but have a similar cosmetic result. It demonstrated a new approach for minimal invasive surgery.
