2008-2012年铜绿假单胞菌的分布及耐药性分析

摘　　要：目的监测2008-2012年我院铜绿假单胞菌的分离情况及对常见抗菌药物的耐药性。方法分析5年来我院分离培养的铜绿假单胞菌的构成比例、科室分布及药敏分析。采用微量肉汤稀释法和纸片扩散法进行药敏试验,根据美国临床和实验室标准协会2012年标准判断结果;采用WHONET5.6软件对耐药数据进行分析。结果 2008-2012年铜绿假单胞菌每年在临床分离菌株中的构成比均呈上升趋势,5年来共分离培养出铜绿假单胞菌1071株,主要来自于痰液、尿液、伤口脓汁等;主要分布于老干部病房、呼吸内科、ICU。铜绿假单胞菌对氨曲南、美罗培南耐药率明显增加,敏感性较高且较为稳定的药物有头孢哌酮/舒巴坦、头孢吡肟、阿米卡星、环丙沙星。结论加强铜绿假单胞菌监测,了解细菌分布和耐药变化,指导临床合理用药,防止耐药菌株传播。Objective The constitutional ratio,distribution and drug resistance of Pseudomonas aeruginosa isolated from our hospital in 5 years were analyzed. Methods Micro-dilution and disk diffusion test were used to study the common antimicrobial resistance. The results were evaluated based on Clinical and Laboratory Standards Institute in 2012. WHONET5.6 was applied for analysis. Results The constitutional ratio of PAE isolated between 2008 and 2012 was on rising trend. 1071 strains PAE were isolated in 5 years that were mainly from sputum and urine and pus. The wards of PAE were mainly distributed from aged ward and geriatric ward and ICU. The resistance against PAE of aztreonam and meropenem rised obviously. Cefoperazone/sulbactam, cefepime, amikacin and ciprofloxacin were showed excellent and steady activity. Conclusion Monitoring of drug resistance to PAE should be strengthened. Bacterial distribution and the change of drug resistance should be investigated in order to direct rational drug usage in clinic and prevent bacterial strain of drug resistance from being transmitted.
