摘 要:目的探讨糖尿病人群估测肾小球滤过率(eGFR)的分布情况及其影响因素。方法以2006—2007年参加开滦集团健康体检的2型糖尿病人群为9396例为观察队列,将研究对象按年龄〈40岁、40~60岁、60—80岁、≥80岁进行分组,以eGFR〈60ml/(min·1.73m。)定义为肾功能不全事件,应用多因素logistic回归模型分析影响发生肾功能不全的因素。结果研究对象的eGFR在年龄〈40岁、40—60岁、60—80岁、t〉80岁组随年龄的增加而降低(F=136.682,P〈0.001);各年龄组共发生肾功能不全者为1841例,年龄〈40岁、40~60岁、60一80岁、i〉80岁组分别为50、749、9125、130例,发生率分别为14.4%、13.8%、27.0%和54.6%(x2=423.431,P〈0.001);Logistic回归分析结果显示,影响发生肾功能不全事件的因素有性别、年龄、收缩压、甘油三酯、血尿酸、血肌酐及尿素氮(P均〈0.05);年龄60~80岁组发生。肾功能不全的危险为年龄〈40岁组的5.50倍,年龄≥80岁组发生。肾功能不全的危险为年龄〈40岁组的82.51倍(P均〈0.05)。结论男性人群、高龄、高收缩压水平、高甘油三酯、高尿酸、高血肌酐及高尿素氮是糖尿病人群发生肾功能不全的影响因素;年龄60~80岁及≥80岁患者发生肾功能不全的风险最大。Objective To investigate the distribution and effective factors of estimated glomerular filtration rate(eGFR) in diabetic patients. Methods A total of 9396 diabetic patients who had participated in the health examination from 2006 to 2007 were selected as the observation cohort. The observation population was divided into four groups according to their age, including the less 40 years group, the 40 - 60 years group, the 60-80 years group and the more than 80 years group. Defined renal insufficiency as eGFR 〈 60 ml/(min. 1.73 m2). Multiple logistic regression analysis was used to analyze the effective factors of renal insufficiency. Results ( 1 ) The eGER of all subjects was decrease as the age increase ( F = 136. 682, P 〈 0. 001 ). (2)There were 1814 subjects with renal insufficiency in the health examinations during 2006 to 2007. There are 50,749,9125 and 130 cases respectively with renal insufficiency at the group of 〈 40 years, 40 - 60 years ,60 -80 years old and 〉180 years old respectively. The incidences of renal insufficiency in the four group was 14. 4%, 13.8% ,27.0% and 54. 6% respectively ( X2 = 423.431, P 〈 0. 001 ). (3) Multiple logistic regression analysis showed that the gender, age, systolic blood pressure, triglyceride, uric acid, creatinine and urea nitrogen were the factors related to renal insufficiency in diabetic patients ( P 〈 0.05 ). ( 4 ) Compared with the group of 〈 40 years, the incident rates of renal insufficiency was 5.50 folds ( 3.69 - 8.20 ) and 82. 51 folds (43.01 - 158.26) respectively at 60 - 80 years group and i〉 80 years old group. Conclusion ( 1 ) Diabetic patients with male, older, higher levels of systolic blood pressure, triglyceride, uric acid, creatinine and urea nitrogen were with high risk of renal insufficiency. ( 2 ) The top risk factors of renal insufficiency was age between 60 - 80 years.
