摘  要:目的观察腰麻剖宫产术中泵注催产素对产妇BP、HR和产后出血等影响。方法择期腰-硬联合麻醉下剖宫产手术产妇120例随机均分为三组,胎儿娩出断脐后分别泵注催产素1U/min(A组)、0.5U/min(B组)和0.33U/min(C组)。记录30min泵注期间BP和HR变化,观察子宫张力、产后出血和不良反应。结果三组患者的收缩压变化之间比较没有差异;但在泵注期间的大部分观测时间点,A、B组舒张压明显低于C组,HR快于C组(P<0.05)。A、B组不良反应明显多于C组(P<0.05)。三组间的产后出血量和米索使用没有统计学差异。结论剖宫产术泵注小剂量(0.33U/min)催产素有利于维持产妇循环稳定和减少催产素相关不良反应。Objective To investigate the effects of oxytocin infusion on blood pressure and heart rate(HR) of patients undergoing selective Cesarean section under spinal anesthesia. Methods A total of 120 parturient undergoing selective Cesarean section was equally divided into three groups, who were infused oxytocin in the rates of 1 U/min(group A),0. 5 U/rain(group B),and 0.33 U/min (group C) after newborns deliveried. The changes of systolic blood pressure (SBP), diastolic blood pressure(DBP) and HR in 30 rain of infusion were recorded and the uterus tension, postpartum blood loss and adverse effects were observed. Results The differences in the changes of SBP during oxytocin infusion were not significant among three groups. The DBP in the most time points observed during oxytocin infusion was significantly lower, HR was quicker, in groups of A and B than those in group C (P〈0. 05). The adverse effects were more in groups of A and B than those in group C(P〈0. 05). There were no significant differences in postpartum hemorrhage and need for additional uterotonics among three groups. Conclusion After newborns deliveried during Cesarean section, oxytoein infusion in small dose(0. 33 U/min) is helpful in maintaining maternal circulation and reducing oxytocin-related adverse reactions.
