特发性肺纤维化合并肺癌与肺癌患者的临床特点分析

摘　　要：目的:分析特发性肺纤维化合并肺癌(IPF-LC)和肺癌(LC)患者的临床特点,为临床医师提供相关信息。方法:2007年4月～2012年4月在新疆维吾尔自治区人民医院住院患者中确诊特发性肺纤维化合并肺癌患者48例和同期确诊肺癌患者114例,对两组患者的发病年龄、吸烟指数、肿瘤分期及生存时间进行分析。结果:IPF-LC组患者平均年龄高于肺癌组患者(P<0.001)。IPF-LC组吸烟指数高于LC组(P=0.003);IPF-LC组患者肺癌以Ⅲ～Ⅳ期为主(P<0.05)。两组患者60岁以上生存时间中位数:IPF-LC组为10.520月,肺癌组为15.743月(P=0.013)。结论:IPF-LC患者中吸烟的老年男性更易发病,且随着年龄的增大,发病机会增高,并与吸烟蓄积量有相关性;合并特发性肺纤维化的肺癌患者分期更差、生存时间更短。Objectives： To analysis idiopathic pulmonary fibrosis in patients with lung cancer and patients of lung cancer clinical data characteristics, to explore the difference of clinical characteristics of patients between the two groups. Methods： Idiopathic pulmonary fibrosis in 48 patients with lung cancer （IPF-LC group） and 114 patients di- agnosed lung cancer （LC group） were enrolled from April 2007 to April 2012 in the Xinjiang Uygur Autonomous Re- gion People＇s Hospital. To analysis difference between the two groups. Results： There were significant differences in pathogenic age between the two groups （P〈0.001）, Mean age of patients with IPF - LC is higher than of the group of LC. Smoking index is significant difference in IPF-LC group and LC group （P=0.003）; Survival time,beyond 60 years old, the median survival time of patients has significant difference between two groups （P= 0.013）.When age greater than 60, patients with IPF-LC survival time less 5.223 months than patients with LC. Conclusion： Oder and smoking men with pulmonary fibrosis are more likely combing with lung cancer, and the duration and quantity of cigarette smoking of patients are related to IPF combine with lung cancer. Tumor staging of patients of idiopathic pulmonary fibrosis with lung cancer is even worse and time of survival was shorter than of patients with lung cancer.
