摘  要:目的:探讨降钙素原(procalcitonin,PCT)、超敏C反应蛋白(high sensitive C-reactive protein,hs-CRP)及白细胞(WBC)计数在肺内细菌感染性疾病中的诊断价值。方法:将221例肺内感染患者根据病原学和血清学诊断结果分为细菌感染组(73例),病毒感染组(47例),支原体感染组(45例),真菌感染组(56例),另选择健康体检者62例作为阴性对照组,分别检测患者血清中PCT、hs-CRP、WBC和中性粒细胞(neutrophil,Neu)的值,比较各项指标组间有无差异,根据受试者工作特征(receiver operating characteristic,ROC)曲线,确定各项指标在肺内细菌感染性疾病中的诊断价值。结果:在肺内细菌感染性疾病中的PCT、hs-CRP、WBC、Neu的ROC曲线下面积(area under the ROC curve,AUC)分别为0.979、0.789、0.691、0.631。结论:PCT、hs-CRP和WBC对于肺内细菌感染均有一定的诊断价值,但PCT是肺内细菌感染诊断的准确性较好的指标,并可用于评估感染的严重程度。Objective: To study the diagnostic value of procalcitonin (PCT), high sensitive C-reactive protein (hs-CRP), WBC in patients with lung bacterial infection. Methods: A sample of 221 patients with lung bacterial infection was divided into four groups, including bacterial infection (73 patients), virus infection (47 patients), mycoplasma infection (45 patients), fungus in- fection (56 patients) according to the results of pathogen and serum. And 62 healthy volunteers was selected as negative control. PCT, hs-CRP, WBC, neutrophil (Neu) were measured respectively and the results were statistically analyzed. The receiver operat- ing characteristic (ROC) curve was used to evaluate the value of PCT, hs-CRP, WBC and Neu. Results: The area under the ROC curve (AUC) of PCT, hs-CRP, WBC, Neu were 0.979, 0.789, 0.691, 0.631respectively. Conclusions: PCT, hs-CRP, WBC and Neu had some value to diagnosis lung bacterial infection. But PCT is a better marker whether there is bacterial infection. The level of PCT can evaluate the severity of bacterial infection.
