痔上黏膜环切吻合术结合外痔切除缝合术在治疗环状嵌顿痔中的临床应用

摘　　要：目的 探讨痔上黏膜环切吻合术（PPH）结合外痔切除缝合术治疗环状嵌顿痔的临床应用.方法 选择PPH结合外痔切除缝合治疗环形嵌顿痔患者35例（治疗组）,另选取同期外剥内扎手术治疗环状嵌顿痔患者35例（对照组）,观察两组手术时间、术后并发症、住院时间及切口愈合时间.结果 两组手术时间及术后尿潴留比较差异无统计学意义（P＞0.05）,两组术后并发症（疼痛评分、水肿评分及排便评分）、术后住院时间、切口愈合时间方面比较差异有统计学意义（P＜0.05）,治疗组治疗效果明显优于对照组.结论 与传统手术相比,PPH结合外痔切除缝合术具有疼痛轻、创伤小、恢复快等优点,是治疗环状嵌顿痔患者理想方法.Objective To investigate the clinical application of PPH combined with external hemorrhoid resection and suture in the treatment of annular incarceration hemorrhoids. Methods Thirty- five patients with annular incarceration hemorrhoids （treatment group）were treated by PPH combined with external hemorrhoid resection and suture, the other 35 cases（ control group） were treated by external dis- section and internal ligation at the same period. Postoperative complications, operational time, hospitali- zation time, and wound healing time of two groups were observed. Results There was no sighifieant difference between the treatment group and the control group in the operational time and retention of urine （ P 〉 0. 05 ）. There were significant differences in incidence of postoperative complications （ pain score, dropsy degree grading and defecation score） , wound healing time and hospitalization time between the two groups （ P 〈 0. 05 ）. The therapeutic effect in the treatment group was better than that in the control group. Conclusions Compared with traditional surgery, PPH combined with external hemorrhoid resection and suture in the treatment of annular incarceration hemorrhoids has the advantages of light pain, less trau- ma, quick recovery and less complication. So it is an ideal treatment of annular incarceration hemorrhoids.
