摘  要:目的了解湖北省重症监护病房ICU医院感染特点,为制定有效的医院感染防控措施提供依据。方法以22所三级医院2010年入住ICU的所有患者为调查对象,结合病历资料和床旁观察情况填写调查表。结果共监测20641例患者,感染例次率6.92%,病人日感染率14.64‰,感染以下呼吸道为主;与留置导尿管、动静脉插管、使用呼吸机相关部位的日感染率分别为1.50‰、1.40‰、30.82‰。医院感染病原菌以鲍曼不动杆菌为主,其次是铜绿假单胞菌、金黄色葡萄球菌。结论 ICU医院感染以下呼吸道居多,呼吸机相关肺炎感染率较高,应及时采取相应干预措施预防控制医院感染。OBJECTIVE Characteristics of nosocomial infections (NIs) in intensive care units (ICUs) in Hubei Province were studied to provide references for the development of effective preventive measures against NIs. METHODS Patients hospitalized at the ICUs of 22 AAA hospitals were chosen as the subjects of this study. Medical records and bedside conditions of the patients were investigated. RESULTS A total of 20 641 patients were surveyed. The case-time infection rate was 6.92%, and the daily infection rate was 14.64%e. Infections occurred primarily in the lower respiratory tract. Daily infection rates of catheter-associated urinary tract infections, central line-associated bloodstream infections, and ventilator-associated pneumonia were 1.50%e, 1.40%e, and 30.82%o, respectively. Acinetobacter baumannii was the most frequently found pathogen in the hospitals; Pseudomonas aeruginosa and Staphylococcus aureus were also common. CONCLUSION Nosocomial infections at ICUs occur primarily in the lower respiratory tract, and the infection rate of ventilator-associated pneumonia is relatively high. Proper preventive measures shall be timely implemented to prevent and control the occurrence of NI.
