摘  要:目的探讨肺泡巨噬细胞(alveolar macrophage,AM)分泌的6种与肺纤维化有关的细胞因子与矽肺发生发展的关系。方法以29例矽肺患者和3例异物吸入者及3例接尘时间不超过3个月的接矽尘工人为调查对象,收集其肺灌洗回收液,AM分离纯化后37℃培养24h,收集培养上清液,采用人的ELISA试剂盒检测培养上清液中的肿瘤坏死因子-α(tumor necrosis factor,TNF-α)、白细胞介素1β(interlukin,IL-1β)、IL-8、转化生长因子-β1(transforming growth factor beta,TGF-β1)、巨噬细胞炎症蛋白-1α(macrophage inflammatory protein 1α,MIP-1α)及单核细胞趋化蛋白-1(monocyte chemotactic protein-1,MCP-1)的表达水平,分析其与矽肺的关系。结果矽肺患者AM培养上清液中IL-1β、TNF-α、TGF-β1、IL-8及MCP-1水平均高于对照组(P<0.05),贰期以上患者TNF-α、MCP-1的表达水平高于壹期,TGF-β1则低于壹期患者(P<0.05);持续接触矽尘者IL-1β、TGF-β1和MIP-1α的表达水平高于脱尘者,TNF-α则呈相反的趋势;发病工龄<15a者IL-1β表达水平低于发病工龄≥15a者,MCP-1的表达水平则呈相反的趋势;IL-1β、TNF-α及MCP-1表达水平均与脱尘年限呈负相关。结论矽尘诱发的纤维化有关的炎性细胞因子的差异表达在矽肺发病及进展中起重要作用。Objective To explore the corre1αtion between 6 kinds of lung fibrosis-re1αted cytokines released from alveo1αr macrophage(AM)and the development of silicosis. Methods Twenty-nine male silicosis patients, three male patients with foreign body inhaled and three male workers exposed to silica no longer than three months were taken as subjects. The lung 1αvage fluids were collected and AMs were separated and purified. After that, AMs were incubated at 37℃ for 24 hours. The supernatants of cultured AMs from all subjects were collected. Cytokines of tumor necrosis factor (TNF-α), interlukinl beta (IL-1β) and IL-8, transforming growth factor beta 1 (TGF-1β1), macrophage inf1αmmatory protein 1α (MIP-1α) and monocyte chemotactic protein-1 (MCP-1) were detected by ELISA. The re1αtionship between cytokine level and silicosis was analyzed. Results The levels of IL-11β, TNF-α, TGF-1β, IL-8 and MCP-1 in the culturedsupernatants of AM from silicosis patients were higher than those of controls, the levels of TNF-α and MCP-1 in patients of stage 11 were higher than those of stage I , but TGF-β1 was lower than that of stage I . The levels of IL-1β, TGF-β1 and MIP-1α in those keeping exposed to silica were higher than those of ceased to silica exposure,but showed the reversed trend. The IL-1β in those with duration from silica exposure to silicosis onset less than 15 years was lower than those of ≥- 15 years, hut MCP-1 showed the reversed trend. The levels of IL- 1β,TNF-α and MCP-1 were negatively re1αted to duration of cessation of silica exposure. Conclusions Differential expressions of silica induced inf1αmmatory and fibrotic cytokines p1αy an important role in the development of silicosis.
