摘  要:目的:观察右归饮协同骨髓干细胞(Mesenchymal Stem Cells,MSCs)转染VEGF(Vascular Endothelial Growth Factor,血管内皮细胞生长因子)移植对激素性股骨头坏死家兔模型骨修复的影响。方法:45只兔随机分为5组:A、B、C、D组为模型组,醋酸泼尼松6周注射建立兔SANFH模型,E组为空白对照组;分离、培养、增殖MSCs,并利用慢病毒载体对MSCs进行VEGF165基因转染;B组进行髓芯减压+MSCs移植,C组进行髓芯减压+VEGF转染MSCs移植,D组进行右归饮灌胃治疗协同髓芯减压+VEGF转染MSCs移植,细胞移植量均1×106。对第8、10、12周的各组一般形态学、血脂变化、股骨头组织病理学和RT-PCR观察VEGF表达情况进行检测。结果:B、C、D组较A组的观测指标均有明显改善,其中血脂指标、空骨陷窝阳性率和VEGF阳性表达各组与A组组间比较差异均有统计学意义(P<0.01),且D组改善优于B、C两组(P<0.05)。结论:右归饮协同转染VEGF的MSCs移植治疗SANFH中能发挥更好的疗效,有较好的改善血运与骨修复重建作用。Objective :To observe the effect of Youguiyin combined with mesenchymal stem cells (MSCs) transfected with VEGF (vascular endothelial growth factor) transplantation for repairing rabbit model of steroid - induced avascular necrosis of femoral head(SANFH). Methods: 45 rabbits were randomly divided into 5 groups: group A, B, C, D were SANFH rabbit model which were established by six weeks injection of prednisone acetate. Group E was the control group; rabbit MSCs were isolated and cultured and then transfected with VEGF165 gene by lentiviral vector(LV). The core de- compression with MSCs transplantation was on group B, core decompression with hVEGF165 - MSCs transplantation on group C,and group D added Youguiyin consecutive treatment with hVEGF165 -MSCs transplantation therapy. The cell transplantation volume was 1 x 106. The general morphology of each group of 8,10,12 weeks, blood lipid changes, histo- pathology of femoral head and RT - PCR were observed and the expression of VEGF were detected. Results : Compared with group A, Group B, C,D showed significant improvement in lipid parameters, positive rates of empty lacunae, and VEGF expression and each group and group A had statistically significant differences( P 〈 O. 01 ), and group D was superior to group B and C(P 〈0.05). Conclusion:Collaborative transplantation of hVEGF165 -MSCs with Youguiyin can get better outcomes in treating SANFH, better improving the blood supply and bone repair and reconstruction role.
