羟苯磺酸钙联合单唾液酸四己糖神经节苷脂钠治疗老年糖尿病周围神经病变疗效的观察

摘　　要：目的观察羟苯磺酸钙联合单唾液酸四己糖神经节苷脂钠治疗老年糖尿病周围神经病变(DPN)的临床疗效。方法将纳入研究的DPN患者分为试验(A,67例)组和对照(B,64例)组。在控制血糖基础上,A组予单唾液酸四己糖神经节苷脂钠加羟苯磺酸钙治疗,B组予甲钴胺注射液治疗,8周后对两组进行有效性评估。结果(1)治疗8周后,A组密歇根糖尿病神经病变评分量表(MDNS)及密歇根神经病变筛查量表(MNSI)评分较B组及治疗前降低(P均<0.05);(2)治疗8周后,A组冷感觉阈值温度(26.33±1.07)℃较B组(23.17±1.21)℃及治疗前(21.64±1.12)℃升高。A组热感觉、冷痛觉阈值温度分别较B组及治疗前降低(P均<0.05);(3)治疗8周后,A组运动神经传导速度(MCV)中正中神经与腓总神经,以及感觉神经传导速度(SCV)中正中神经与腓总神经均较B组及治疗前升高(P均<0.05)。结论单唾液酸四已糖神经节苷脂钠联合羟苯磺酸钙治疗老年DPN疗效较好。Objective To observe the efficacy of calcium dobesilate dispersible plus monosialotetrahex-osylganlioside sodium on elderly diabetic peripheral neuropathy （DPN）patients. Methods The 157 DPN patients, aged ≥ 60 years, were randomized into the experiment group （A,n= 67） and the control group （B,n=64）. All subjects were basically treated for plasma glucose control. The A group was treated with monosialotetrahex-osylganlioside sodium and calcium dobesilate dispersible. The B group was treated with methylcobalamin injection. After 8 weeks＇ treatment, the assessment of therapeutic effect was done for evaluation. Results After 8 weeksr treatment, （1） the Michigan Diabetic Neur0pathy Score （MDNS） and Michigan Neuropathy Screening Instrument （MNSI） scores in the A group were significantly lower than in the B group and than before treatment （P〈0. 05） ; （2） the cold sensation threshold of the A group （26.33±1.07）℃ was significantly higher than the B group （23.17± 1.21）℃ and before treatment （21.64 ±1.12）℃, while the thermal sensation and cryalgesia of the A group were significantly lower than the B group and than before treatment; and （3） the motor nerve conduction velocity （MCV） and sensory nerve conduction velocity （SCV） of median nerve and peroneal nerve were significantly higher in the experiment group than in the control group and than before treatment （P〈0. 05）. Conclusion A good therapeutic effect is achieved in the treatment of elderly DPN patients with calcium dobesilate dispersible and monosialotetrahex-osylganlioside sodium in combination. It is worth spreading in clinical practice.
