摘 要:目的分析冠状动脉旁路移植术(CABG)医院感染发生情况,为进一步研究感染相关危险因素及干预措施提供依据。方法对北京市医院感染监控系统3年10所医院CABG目标性监测数据医院感染病例进行统计分析。结果 3年CABG手术监测9375例,发生医院感染241例、293例次,感染率为2.57%、例次感染率为3.13%;医院感染首位为下呼吸道感染,其次为手术部位感染(SSI);CABG感染病例平均住院天数延长19.85d,高于北京市同期其他清洁手术医院感染导致的住院日增加;年龄>50岁、住院>10d、手术时间>3h的患者为医院感染高危人群。结论 CABG成败关键之一是医院感染控制,在术后肺炎、SSI、血液感染等医院感染控制中,仍有很多环节需要持续改进。OBJECTIVE To analyze the incidence of nosocomial infections in patients undergoing coronary arterybypass grafting surgery (CABG) so as to further study the related risk factors of the infections and put forwardthe intervention measures. METHODS The patients who underwent CABG procedures in 10 medical institutions inBeijing were enrolled in the study,and the targeted monitoring data in the past three years were statisticallyanalyzed. RESULTS Of totally 9375 cases of CABG under the monitoring in three years, the nosocomial infectionsoccurred in 241 cases of patients (293 case-times) with the infection rate of 2. 57% and the case-time infection rateof 3. 13%. The lower respiratory tract was the predominant infection site, followed by the surgical site; theaverage hospitalization duration of the CABG cases extended to 19. 85 days, higher than that of the cases whosehospitalization duration were due to the cleaning surgery leading nosocomial infections. The patients with morethan 50 years of age,hospitalization duration more than 10 days, or operation duration more than hours were thepopulation at high risk of the nosocomial infections. CONCLUSION It is crucial for the CABG to control thenosocomial infections, and there are many links that needed to be continuously improved during the control ofpostoperative pneumonia, SSI, and the blood infections.
