摘  要:目的研究分析复杂性尿路感染病原菌分布及抗菌药物的临床应用,以降低感染率。方法选取2009年1月-2012年1月复杂尿路感染的住院患者186例,收集其清洁中段尿进行细菌培养,并将分离出的200株病原菌进行药敏试验,分析病原菌的分布及耐药性,总结抗菌药物的合理应用方法。结果 200株病原菌检出革兰阴性杆菌138株占69.0%,其中大肠埃希菌占40.0%,革兰阳性球菌感染55株占27.5%,真菌感染7株占3.5%;耐药率<30.0%的抗菌药物有阿米卡星、氨苄西林/克拉维酸、头孢曲松、头孢噻肟、氨曲南、头孢哌酮、亚胺培南、美罗培南。结论复杂性耐药菌的感染仍以革兰阴性菌尤其是大肠埃希菌为主,并且对常用抗菌药物的耐药性较为严重,临床应及时调整用药方案。OBJECTIVE To investigate the distribution of pathogenic bacteria of complicated urinary tract infectionsand the clinical application of antibacterial drugs so as to reduce the infection rate. METHODS 186 cases of patientswith complicated urinary tract infections enrolled in our hospital from Jan 2009 to Jan 2012 were chosen as the research subjects. The patients' clean catch midstream urine was collected for bacterial culture, and the isolated 200strains were subjected to drug sensitivity testing, the distribution of pathogenic bacteria and their drug resistancewere analyzed and reasonable application of antibacterial drugs was summarized. RESULTS Of totally 200 strainsof pathogens isolated there were 138 strains gram-negative bacilli(69. 0%),including Escherichia coli (40. 0%),and 55 strains of gram-positive cocci(27. 5%) and 7 strains of fungi(3. 5%). The drug resistance rates to amika-cin, ampicillin/clavulanic acid,ceftriaxone, cefotaxime,aztreonam, cefoperazone, imipenem,and meropenemwere less than 30. 00?. CONCLUSION The gramnegative bacteria especially Escherichia coli are the major complicated drug-resistant bacteria,which are highly to the common antibacterial drugs. It is necessary for the hospital to adjust medication program in a timely manner.
