白细胞VCS参数在细菌及病毒感染的鉴别临床应用研究

摘　　要：目的研究细菌感染及病毒感染白细胞VCS参数的变化特点,探讨白细胞VCS参数在感染性疾病病原体的鉴别中的临床应用价值。方法 89例细菌感染患者为细菌组,117例病毒感染患者为病毒组,另以59名健康体检人员为对照组,研究3组白细胞VCS参数变化特征,包括中性粒细胞平均体积(NEV)、中性粒细胞平均传导值(NEC)、中性粒细胞平均散射值(NES)、淋巴细胞平均体积(LYV)、淋巴细胞平均传导值(LYC)、淋巴细胞平均散射值(LYS)、单核细胞平均体积(MOV)、单核细胞平均传导值(MOC)及单核细胞平均散射值(MOS)。结果细菌组中性粒细胞平均体积(NEV)为149.1±9.9、淋巴细胞平均传导值(LYC)为109.4±4.1、单核细胞平均体积(MOV)为169.9±9、中性粒细胞平均传导值(NEC)为147.7±4.5升高,单核细胞平均传导值(MOC)为123.4±4.2,均有所降低,其他变化不明显,病毒组LYC 103.5±3.8、MOV 167.2±4.3升高,LYC 103.5±3.8、MOC124.3±2.7降低,其他变化不明显;细菌组中白细胞>15×109/L组别NEV、NEC、LYV、MOV升高程度高于白细胞<15×109/L组。结论白细胞VCS参数能较好地反应感染状态白细胞形态、结构的变化,对细菌及病毒感染的鉴别有一定的辅助作用。OBJECTIVE To study the characteristics of change of leukocyte VCS parameters in the patients withbacterial infections or viral infections and explore the value of the leukocyte VCS parameters in identification ofpathogens causing infectious diseases. METHODS Totally 89 cases of patients defined as bacterial infections werechosen as the bacterial group, while 117 cases of patients with viral infections were set as the virus group, and 59healthy people were assigned as the control group, the characteristics of change of white blood cells VCS parameters, including the NEV, NEC, NES, LYV, LYC,LYS, MOV,MOC, and MOS, were observed between thethree groups. RESULTS In the bacterial group, the NEV was （149. 1 ±9. 9）,LYCC109. 4±4. 1）,MOV（169. 9±9） , NEC（147. 7±4. 5） , which was increased,MOC （123. 4±4. 2）,which was decreased, while other parametersdid not change significantly; in the virus group; the LYC was （103. 5±3. 8）,MOV（167. 2±4. 3） , which were in-creased, the LYC（103. 5± 3. 8）, MOC（124. 3 ± 2. 7）,which were decreased,while other parameters did notchange significantly. In the bacterial group,the NEV,NEC,LYV,and MOV of the groups with the white bloodcell counts more than 15×10^9 /L were higher that those with the white blood cell counts less than 15×10^9/L.CONCLUSION The leukocyte VCS parameters can be a better response to the white blood cell morphology at theinfection status and the change of structure, helpful to the differentiation between the bacterial infection and theviral infection.
