摘  要:目的 探讨5~14岁健康儿童肺通气功能状况,确定北京地区儿童肺功能正常值范围和正常预计值方程.方法 选择幼儿园和中小学5 ~14岁健康儿童426例(男213例,女213例),采用德国Jaeger肺量仪进行流量-容积曲线和最大每分钟通气量(MVV)测定,采用便携式峰流速仪进行最大呼气峰流速(PEFR)测定.所选肺功能参数包括用力呼气肺活量(FVC)、0.5s用力呼气容积、1 s用力呼气容积(FEV1)、FEV1/FVC、最大呼气中段流速、用力呼气峰流速、用力呼出25%肺活量最大流速、用力呼出50%肺活量最大流速、用力呼出75%肺活量最大流速、用力呼气时间、MVV、PEFR等.对以上参数进行多元逐步线性回归分析,获得回归方程式.结果 北京市5 ~ 14岁儿童随年龄增加,各肺功能参数均明显增加(P均<0.01).<12岁儿童,不同性别间主要肺功能参数差异无统计学意义,但在13岁、14岁两年龄组男童PEFR、FVC、FEV1、MVV均显著高于女童(P均<0.01).肺功能各参数与年龄、身高、体质量呈正相关,其中与身高相关性最强,其次为年龄、体质量.建立了各项肺功能参数的多元回归方程.结论 确定北京市5~ 14岁儿童的流量-容积曲线各主要参数、PEFR、MVV的正常值,并建立了其预计值方程,为临床医师准确判断肺功能异常奠定基础.Objective To assess the lung function of healthy children aged 5 to 14 years and to determine the spirometric reference values and predicted value equations of children living in Beijing.Methods Four hundred and twenty-six healthy children (213 boys and 213 girls) aged 5 to 14 years from kindergartens and primary schools in Beijing were included in the study.Tests of lung function were carried out in all children.The tests included measurement of flow-volume curve,maximal per minute ventilation (MVV) and peak expiratory flow rate (PEFR) by using Jaeger Masterscreen spirometry machine and portable peak flow meter.Lung function parameters including forced vital capacity (FVC),forced expiratory volume in 0.5 second,forced expiratory volume in 1 second (FEV1),FEV1/FVC,maximal mid-expiratory flow rate,peak expiratory flow,forced expired flow at 25% of FVC,forced expired flow at 50% of FVC,forced expired flow at 75% of FVC,forced expiratory time,MVV,and PEFR were recorded.Stepwise multiple linear regression was adopted to derive the regression equations.Results The value of lung function parameters increased significantly along with the increasing of age among children aged 5 to 14 years (all P <0.01).No significant difference was found in values of major lung function parameters between males and females except for the 13 and 14 age groups.The lung function values of PEFR,FVC,FEV1,MVV were significantly higher in boys than those in girls aged 13 and 14 years (all P < 0.01).Lung function level was positively correlated with age,height and weight.The strongest correlation was found between FEV1 and height which was followed by age and weight.Reference values and regression equations of children aged 5 to 14 years in Beijing were established.Conclusions Reference values of major lung function parameters by PEFR,MVV were determined,and the predicted value equations were established in children aged 5 to 14 years in Beijing,which would help the pediatric clinicians to make accurate eva
