肝移植术中血浆制品输注影响因素的分析研究

摘　　要：目的通过回顾分析,确定影响肝移植术中血浆制品输注因素。方法根据术中血浆制品输注量,对127例肝脏移植受者分为3组,比较各组手术前及术中资料,逐步回归选择预示术中血浆制品输注指标。结果不同输注组间的术前Hb、Plt、ChildPugh评分、ALB、PA、TBil、PT和APTT有显著差异,术前APTT和术中静脉补液量、失血量及红细胞悬液输注量对术中血浆制品的输注量有预示作用。结论影响肝移植术中血浆制品输注的主要因素为术中静脉补液、血液丢失和红细胞悬液输注,患者肝脏病变的严重程度和凝血紊乱影响有限。Objective To define the factors intraoperative plasma products transfusion in patients underwent liver transplantation by retrospective data analysis. Methods According to the requirements of plasma products transfusion, 127 patients who received liver transplantation were divided into 3 groups. Preoperative and intraoperative data were compared between groups. The predictive index for intraoperative plasma products transfusion were selected by stepwise regres sion. Results There were significant differences between groups in concentration of patients＇ preoperative hemoglobin, platelet, ChildPugh scores, serum albumin, serum prealbumin, total bilirubin, prohrombin time, and activated partial thrombo plastin time. Preoperative activated partial thromboplastin time and the amount of fluid infusion, blood loss and red blood ceils transfusion during operation have predictive values for intraoperative plasma transfusion. Conclusion The main fac tors that influence intraoperative plasma products transfusion were the amount of fluid infusion, blood loss and red blood cells transfusion during the operations. The serious impairment of patient＇s liver function and coagulation disorder have lim- ited effects on the requirement of intraoperative plasma products.
