TPOAb在老年结节性桥本氏甲状腺炎诊断中的价值

摘　　要：目的:探讨TPOAb在老年结节性桥本氏甲状腺炎中的诊断价值。方法:选取2008年1月-2010年12月于本院内分泌科、普外科及老年科发现的老年甲状腺结节患者148例,其中96例结节性甲状腺肿患者、52例结节性桥本氏甲状腺炎患者。同时选取本院体检中心健康体检者32例作为健康对照。通过化学发光免疫分析法检测各组患者血清中TPOAb浓度,比较各组之间阳性率的差异。结果:结节性桥本氏甲状腺炎患者中TPOAb的阳性率显著高于结节性甲状腺肿患者和健康人群,比较差异均有统计学意义(P<0.05)。结论:在老年结节性桥本氏甲状腺炎患者中TPOAb的水平显著升高,具有较高的特异性,但其敏感性较低,需联合其他指标(如TGAb)以提高诊断的敏感性。Objective： To study the diagnostic value of thyroid peroxidase antibody in old Hashimotos thyroiditis patient.Method： 96 old patients with struma nodosa, 52 old patients with nodular Hashimotos thyroiditis were selected from endocrinology department, general surgery department and gereology department.32 cases of healthy subjects were selected from medical examination center.Serum thyroid peroxidase antibody were determined by chemiluminescent immunoassay, the positive rate between the groUps was compared.Result： Compared with stroma nodosa group and control group, the positive rate of thyroid peruxidase antibody were markedly higher, there were statistically significant difference.Conclusion： In old hashimotos thyroiditis patients, the level of＇thyroid peroxidase antibody is significantly increased.The specificity is high, but the sensibility is low.It should be combined with other items （ such as TGAb ） to increase the diagnosis sensibility.
