含氯法齐明联合方案治疗耐多药结核疗效分析

摘　　要：目的评价含氯法齐明联合方案治疗耐多药结核病的效果。方法对28例耐多药复治肺结核病病人使用含氯法齐明联合方案进行抗结核治疗,每月监测痰涂片找抗酸杆菌及结核菌培养。治疗后12个月细菌学检结果均为阴性为治愈。观察其治疗效果,并分析治疗失败与纤维病灶面积/总病灶面积比值的相关性。结果自身对照显示含氯法齐明联合方案对耐多药肺结核治愈率78.6%,优于一线抗结核药物联合方案差异有统计学意义(P<0.05)。纤维病灶面积/总病灶面积<50%组治疗成功率高于≥50%组,差异有统计学意义(P<0.01)。结论含氯法齐明联合方案对耐多药结核病有较好疗效。Objective To investigate the efficacy of combination treatment with Clofazimine and other antituberculosis drugs for patients with multi-drug resistant tuberculosis.Methods 28 cases with multi-drug resistant tuberculosis which had been failure again by treatment with Isoniazid,Rifampicin,Ethambutol,and Pyrazinamide,were treated again with Clofazimine and other antituberculosis drugs,and monitorred with sputum smear and mycobacterium cuture to find Mycobacterium monthly.The curing means that the bacteriology results were all negative in the last 12 months in the treatment course of all 24 months.The efficacy after treatment and the correlation with fiber focus of infection area / total focus of infection area were assessed.Results The efficacy of combination treatment with Clofazimine and other antituberculosis drugs was better than that of combination treatment with Isoniazid,Rifampicin,Ethambutol,and Pyrazinamide（22/28,78.6%;0/28,0%）;the difference was statistically significant by self contrast（χ2=4.6667,P0.05）.The success ratio in growp with fiber focus of infection area / total focus of infection area 50% was higher than that in growp with fiber focus of infection area / total focus of infection area ≥50%,the difference was statistically significant（P0.01）.Conclusion Combination treatment with Clofazimine and others for patients with multi-drug resistant tuberculosis gains better effect,which can significantly increase the sputum negative conversion rate.
