摘 要:目的评估GenoType MTBDRplus(对结核杆菌复合群利福平和/或异烟肼耐药的分子诊断试剂盒)快速检测耐多药结核病的临床效果。方法收集河南省胸科医院2010年3月-2012年3月收治的结核患者150例,纳入者连续3 d提供痰标本,对每个痰标本进行抗酸杆菌涂片检测,改良罗氏(L-J)固体和MGIT960液体培养,培养阳性菌株采用L-J比例法、MGIT960液体法、GenoType MTBDRplus三种不同方法进行一线抗结核药物敏感性检测,对同一过程的不同试验方法的检测结果两两对比分析。结果 GenoType MTBDRplus检测耐多药结核菌需时短,与L-J比例法、MGIT960液体法对比,三种不同的药敏试验方法对相同菌株的药敏结果一致性两两对比分析差异无统计学意义(P>0.05)。耐多药检测符合率90%。结论GenoType MTBDRplus可用于耐多药结核菌(MDR-TB)的快速检测,满足临床对MDR-TB进行早期诊断和及时治疗的需要。Objective To evaluate the clinical effectiveness of GenoType MTBDRplus for rapid detection of multi - drug resistant tuberculosis. Methods One hundred and fifty tuberculosis cases were collected from March 2010 to March 2012 in Henan Chest Hospital, and sputum samples were collected from these cases for three consecutive days for acid- fast bacillus smear detection. Then the positive strains were cultured by modified (L- J) solid and MGIT960 liquid culture method, and euhure- positive strains were detected MDR- TB by L- J ratio method, MGIT960 liquid method and GenoType MTBDRplus method. Results GenoType MTBDRplus was rapid in detec- ting multidrug - resistant tuberculosis, when compared to L - J proportion method and MGIT960 method. Results of three kinds of different drug susceptibility testing methods were consistent on the same strain, with the coincidence rate of 90%. Conclusion GenoType MTBDRplus can be used for the rapid detection of MDR - TB, enough for the clinical need of early diagnosis and timely treatment.
