摘 要:目的探讨重组人促红细胞生成素(rhEPO)对大鼠急性缺血肢体股神经的保护作用及意义。方法建立大鼠急性下肢缺血模型,随机分为实验组及对照组,每组设置不同时间点亚组。实验组行腹腔注射1000 IU/kg rhEPO,对照组给予相同体积的生理盐水,于术后第6 h,1 d,2 d,3 d,对股神经进行神经电生理和原位末端标记技术(TUNEL)分析,评价股神经缺血损伤后rhEPO对神经凋亡及功能恢复的影响。采用t检验及单因素方差分析比较两组及不同时间点股神经电生理指标的差异性。结果实验组股神经的阈强度分别在6 h(0.008±0.005)、2 d(0.180±0.011)、3 d(0.022±0.0189)明显优于对照组,其差异有统计学意义(P<0.05)。两组指标随着时间的变化差别有统计学意义(F=4.524,P=0.014;F=7.494,P=0.001)。实验组股神经的最大刺激强度在各个时间点[6 h(0.219±0.088),1 d(0.200±0.042),2 d(0.296±0.082),3 d(0.358±0.025)]均优于对照组[6 h(47.778±10.472),1 d(37.055±3.270),2 d(34.388±4.484),3 d(29.657±7.940)],其差异有统计学意义(P<0.05)。两组指标随着时间的变化差别有统计学意义(F=13.832,P<0.001;F=4.478,P=0.015)。rhEPO组股神经传导速度在各个时间点[6 h(72.225±13.607),1 d(58.335±9.131),2 d(53.112±6.690),3 d(52.557±11.667)]均优于NS组[6 h(47.778±10.472),1 d(37.055±3.270),2 d(34.388±4.484),3 d(29.657±7.940)],其差异有统计学意义(P<0.05)。rhEPO组内各时间点的传导速度差别无统计学意义(P>0.05),NS组传导速度随时间的变化差别有统计学意义(F=9.482,P<0.001;F=1.073,P=0.383)。rhEPO组股神经细胞凋亡率分别在6 h(0.071±0.020)和2 d(0.143±0.035)明显优于NS组[6 h(0.186±0.085),2 d(0.366±0.054)],差别有统计学意义(P<0.05),rhEPO组股神经细胞凋亡率明显低于NS组,两组指标随着时间的变化差别有统计学意义(F=35.527,P<0.001;F=24.760,P<0.001)。结论 rhEPO是一种有效的神经保护剂,早期使用rhEPO可以有效减少缺血神经元的凋亡Objective To investigate the neurprotective effect of recombined human erythropoietin (rhEPO) on femoral nerve of acute ischemic limb, which using. Methods Acute lower limb ischemia animal model was established using wistar rats, and they weredivided randomly into experimental group and control group. Each group was divided into four subgroups according to different time points. Rats were treated subcutaneously with 1000 IU/kg of rhEPO in rhEPO group and same volume of normal saline in control group after the operation at six hours, one day, two days and three days. Electro-physiological evaluation and TUNEL assay were performed at 6 hours, 1 day, 2 days and 3 days postoperatively respectively to evaluate the influence of rhEPO on neurons apoptosis and the functional recovery after femoral never ischemia. Analysis of Variance and t test was used for statistical analysis. Results At the time points of 6 hours, 2 days and 3 days, the threshold intensity of femoral nerve of experimental group [6 h (0.008 ± 0.005), 2 d (0.180 ± 0.011), 3 d (0.022 ± 0.0189)] were lower than the control group, and the difference was statistically significant (P 〈 0.05 for all). According to single factor analysis of variance, index were statistically significant as the change of time between the two groups (F = 4.524, P = 0.014; F = 7.494,P = 0.001). The maximum stimulation intensity of femoral nerve of rhEPO group at each time [6 h ( 0.219 ± 0.088), 1 d (0.200 ± 0.042), 2 d (0.296 ± 0.082), and 3 d (0.358 ± 0.025) resctively] were lower than the NS group [6 h (47.778 ± 10.472), 1 d (37.055 ± 3.270), 2 d (34.388 ± 4.484), and 3 d (29.657 ± 7.940) respectivly]. The difference was statistically significant (P 〈 0.05).
