阴阳经联合针刺治疗亚急性脑卒中的临床疗效

摘　　要：目的探讨针刺对亚急性期脑卒中患者肢体功能的影响。方法采用单盲、分层、区组随机对照的方法,将纳入的110例脑卒中患者采用分配隐蔽的方法随机分为针刺组和对照组。针刺组(55例)接受针刺及神经内科常规治疗,对照组(55例)接受神经内科常规治疗,针刺5 d为1疗程,连续4个疗程。对治疗前及治疗后Fugl-Meyer评定法(FMA),巴氏指数(BI),美国国立卫生研究院卒中量表(NIHSS)评分以及随访期3、6个月末病死/残疾率,复发率进行疗效评估。结果两组基线比较无统计学意义(P>0.05);治疗后NIHSS、FMA、BI评分两组治疗后与治疗前相比均有明显改善(P<0.01);治疗后NIHSS、FMA、BI评分针刺组与对照组相比差异有统计学意义(P<0.05);随访期3、6个月末病死/残疾率,复发率两组比较差异无统计学意义(P>0.05)。结论针刺可明显改善亚急性期脑卒中患者的肢体运动功能、神经功能缺损程度及日常生活活动能力。Objective To explore the effects of Yin and Yang acupuncture in subacute stroke patients.Methods The singleblind,layered,block randomized method was used.110 stroke patients enrolled in the study using the method of allocation concealment were randomly divided into acupuncture and control groups.55 cases in acupuncture group were accepted acupuncture and neurological routine treatment,Acupuncture five days were a course of treatment,four consecutive courses were treated in treatment.Cases in control group were accepted neurological routine treatment.Efficacy assessments included Fugl-Meyer assessment（FMA）,Barthel index（BI）,National Institutes of Health Stroke Scale（NIHSS） scores before and after the treatment and the mortality/disability,recurrence rate at the end of 3 and 6 months follow-up.Results There were no significant differences between the two groups of baseline （P ＞ 0.05）,The scores of FMA,BI,NIHSS after treatment were obviously improved in comparison with those before treatment in both groups （P ＜ 0.01）.There were statistical differences between the two groups in the scores of FMA,BI,NIHSS after treatment（P ＜0.05）.There were no significant differences between the two groups in the mortality/disability,recurrence rate at the end of 3 and 6 months follow-up （P ＞ 0.05）.Conclusions Acupuncture can obviously improve limb movement function,nerve function damage degree and activities of daily living.
