摘 要:目的 探讨行为回放式宣教指导慢性阻塞性肺疾病(COPD)患者氧气雾化吸入对排痰效果的影响.方法 将172例COPD患者随机分为观察组和对照组,每组各86例.两组均行在药物治疗和常规护理的基础上行氧气雾化吸入,观察组采用行为回放式宣教指导患者雾化吸入,对照组采用常规宣教指导患者雾化吸入,比较两组患者氧气雾化吸入后排痰效果.结果 雾化吸入前两组患者呼吸频率、血氧饱和度比较差异无统计学意义(P>0.05);雾化吸入后观察组患者呼吸频率为(19.38±1.19)次/min,对照组为(20.61±1.61)次/min,两组比较差异有统计学意义(t=3.81,P<0.01);雾化吸入后观察组患者Sp02改善值为(4.46±1.26)%,对照组为(2.61±1.04)%,两组比较差异有统计学意义(t=4.382,P<0.05).两组患者痰鸣音消失时间、感染控制时间比较差异均有统计学意义(P<0.05).结论 行为回放式宣教指导COPD患者氧气雾化吸入有利于COPD患者掌握正确的氧气雾化吸入操作方法,激发患者的学习兴趣,提高患者遵医行为,提高护士宣教效率,规范宣教内容,从而使痰液顺利排出,利于肺部感染控制、改善低氧血症,延缓COPD的进展.Objective To investigate the effect of oxygen atomization inhalation on eliminating phlegm of CODP patients through medical education and instruction in the way of behavior replay.Methods 172 COPD patients were randomly divided into observation group and control group,86 patients in each group.Two groups of patients were given oxygen atomization inhalation on the basis of drug therapy and conventional care,the observation group used the way of behavior playback education and instruction,while the control group adopted routine way to education and instruct the patients.Accordingly,the effect of oxygen atomization inhalation on eliminating phlegm of CODP patients of the two groups were compared.Results There were no significant differences in the two groups of respiratory frequency,oxygen saturation before the intervention(P 〉 0.05).After oxygen atomization inhalation,respiratory frequency in observation group was statistically less than that in control group [(19.38 + 1.19)times/min vs(20.61 + 1.61)times/min],and the difference was statistically significant (t =3.81,P 〈 0.01).And the improvement value of oxygen saturation in observation group after treatment was significantly lower than that in the control group [observation group (4.46 ± 1.26%),control group(2.61 ± 1.04%) ; t =4.382,P≤0.01].The two group patients' sputum stridor disappeared time and infection control time had significant difference (P 〈 0.05).Conclusions The behavior playback education and instruction method is help for COPD patients to master the correct operation method of oxygen atomizing inhalation,stimulate the learning interests of the patients,improve patient compliance,enhance the efficiency of nurse's education and instruction,regulate content of education and instruction.Thus,the study can make the patents eliminate phlegm smoothly,raise lung infection control level,improve hypoxemia and delay the progress of COPD.
