不同时期支气管扩张症患者血清降钙素原、C反应蛋白、白细胞水平及意义

摘　　要：目的分析降钙素原(PCT)、超敏C反应蛋白(hsCRP)、白细胞在不同时期支气管扩张患者的血清表达水平及其临床意义。方法随机入组2011年1月至2013年3月本科收治的支气管扩张急性加重期及治疗后缓解期患者30例,行诱导痰细菌培养、血清PCT水平、hsCRP、血白细胞检测,并行APACHⅡ评分评估病情。结果 30例急性加重期支气管扩张患者,有18例(60.0%)痰细菌培养阳性;急性加重期支气管扩张患者血PCT、hsCRP、白细胞水平明显高于治疗后缓解期患者;且急性加重期痰培养细菌结果阳性者血浆PCT、hsCRP水平明显高于培养结果阴性者(P<0.01);痰培养细菌结果阳性者与阴性者白细胞数差异无统计学意义(P>0.05);相关分析表明患者血清PCT水平与APACHⅡ评分呈正相关(r=0.521,P<0.05),hsCRP及白细胞数与APACHⅡ评分无明显相关。结论支气管扩张患者不同时期血清血PCT、hsCRP、血白细胞含量不同,血PCT水平升高可能与细菌感染有关,并与病情严重程度相关。Objective To investigate changes of serum procalcition（PCT）, CRP levels and WBC count in patients with bronchiectasis, and investigate the clinical significance. Methods 30 patients with acute exacerbations of bronchiectasis were studied between January 2011 to March 2013 in our hospital. Got the morning sputum for gemiculture, serum hsCRP, PCT concentrations and white blood cell concentrations were measured. APACH Ⅱ score were used to evaluate the severity of the patients. Results The serum levels of PCT, hsCRP and WBC count in the acute exacerbation of bronchiectasis were both higher than those in the remission stages of bronchiectasis, and the levels of hsCRP , PCT were significantly higher in patients with bronchiectasis when they had bacteria infection compared to those without bacteria infection. PCT levels were positively correlated with APACH Ⅱ score（r=0.521, P〈0.05）,there was no correlative relationship between hsCRP , white blood cell concentrations and APACH Ⅱ score. Conclusion Especially has bacterial infection, the levels of PCT elevation significantly in patients with bronchiectasis. The PCT levels are more closely correlated with severity score.
