摘 要:目的:探讨夫妇双方人类白细胞抗原(Human leukocyte antigen,HLA)-DQ等位基因多态性与不明原因复发性流产之间的关系。方法:对25例不明原因复发性流产夫妇和16例正常妊娠的夫妇,采用序列特异性引物PCR和基因测序分型的方法分别检测夫妇双方HLA-DQA1和HLA-DQB1基因型,并对双方HLA-DQ的基因频率和相容性进行统计学分析。结果:女方不明原因复发性流产患者中HLA-DQA1和HLA-DQB1等位基因频率与对照组相比无统计学差异(P>0.05);在男方患者中,HLA-DQB1*03:01频率显著升高(χ2=4.02,OR=3.29,P=0.045),HLA-DQB1*05:02频率显著降低(χ2=6.38,OR=0.15,P=0.025)。两组夫妇间HLA-DQ位点的匹配个数之间无统计学差异(P>0.05)。结论:男方HLA-DQB1基因位点在不明原因复发性流产发生中起重要作用,HLA-DQB1*03∶01可能是易感基因,而HLA-DQB1*05:02可能是保护性基因。Objective:To investigate the correlation of human leukocyte antigen(HLA)-DQ allele polymorphism and unexplained recurrent miscarriage(uRM).Methods:HLA-DQA1 and HLA-DQB1 genotyping was carried out in 25 couples with uRM(uRM group) and 16 fertile couples(control group),using polymerase chain reaction sequence specific primer(PCR-SSP) and sequence based typing(SBT) methods.The frequency and sharing of HLA-DQ alleles were analyzed by SPSS 16.0 software.Results:In females,the distribution of HLA-DQA1 and DQB1 alleles of uRM group were similar to those of control group(P0.05).But in males,the frequency of HLA-DQB1 * 03:01 allele was significantly increased(χ2= 4.02,OR = 3.29,P = 0.045) in uRM group,whereas the frequency of HLA-DQB1* 05:02 allele was significantly decreased(χ2= 6.38,OR = 0.15,P = 0.025).Furthermore,the HLA-DQ sharing between the two groups was no significant difference.Conclusion:HLA-DQB1 alleles might be partly response for the pathogenesis of uRM,especially for men.DQB1* 03:01 allele may be a susceptible gene,while DQB1* 05:02 allele may be a protective gene for uRM.
