摘 要:目的:分析精液常规参数对卵细胞浆内单精子显微注射(intracytoplasmic sperm injection,ICSI)治疗结局的影响。方法:对290例ICSI治疗周期进行回顾性分析,根据精子参数分为:A(精液常规正常和轻度异常)、B(严重少精)、C(严重弱精)、D(严重畸精)和E(严重混合精子异常)5组。比较各组的受精率、卵裂率、可移植胚胎率、优质胚胎率、着床率和妊娠率是否存在差异。结果:5组的受精率、卵裂率、可移植胚胎率和优质胚胎率比较,差异无统计学意义(χ2=1.27,P=0.87;χ2=1.34,P=0.86;χ2=0.56,P=0.97;χ2=1.39,P=0.85)。D组和E组的生化妊娠率、着床率和临床妊娠率均低于A组(χ2=12.1,P=0.00;χ2=10.15,P=0.00;χ2=10.78,P=0.00;χ2=9.90,P=0.00;χ2=8.13,P=0.00;χ2=11.78,P=0.00)、B组(χ2=6.04,P=0.01;χ2=7.79,P=0.01;χ2=7.21,P=0.01;χ2=4.29,P=0.04;χ2=5.63,P=0.02;χ2=7.21,P=0.01)和C组(χ2=7.01,P=0.01;χ2=6.47,P=0.01;χ2=6.95,P=0.01;χ2=5.14,P=0.02;χ2=4.35,P=0.04;χ2=7.01,P=0.01)。结论:不同参数的精子不影响ICSI治疗的受精结局和胚胎质量,严重形态异常的精子行ICSI治疗后着床率和妊娠率降低。Objective:To evaluate the effects of sperm with different parameters on the outcomes of intracytoplasmic sperm injection (ICSI). Methods :Totally 290 ICSI cycles were analyzed retrospectively. They were divided into 5 groups according to sperm parame ters:group A (normal sperm forms or mild oligoastheno-teratozoospermia), group B (severe oligozoospermia), group C (severe astheno zoospermia), group D (severe teratozoospermia), group E ( severe oligoastheno-teratozoospermia). Fertilization rate, cleavage rate, transferable embryos rate, good quality embryos rate, implantation and pregnancy rates were compared between the groups. Results: Fertilization rate, cleavage rate,transferable embryos rate and good quality embryos rate were not significantly different among the 5 groups (x2= 1.27,x2=-0.87; X2= 1.34, P=-0.86; X2=0.56, P=0.97; 2,2= 1.39,P=-0.85). The chemical pregnancy rate, embryo implantation rate and clinical pregnancy rate were lower in group D and E than in group A (X2=12.1, P=0.00;)(2=10.15,P=0.00;x2= 10.78,P=0.00; x2= 9.90, P=0.00 ; X2=8.13, P=0.00; x2=l 1.78, P=0.00), B (X2=6.04, P=0.01 ; )(2=7.79, P=0.01 ; X2=7.21, P=0.01 ; )(2=4.29, P=0.04; )(2=5.63, P=0.02 ; X2=7.21 ,P=0.01 )and C (X2=7.01 ,P=0.01 ; X2=6.47, P=0.01 ; X2=6.95 ,P=0.01 ; X2=5.14,P=0.02 ; X2=4.35 ,P=0.04 ; X2=7.01 ,P= 0.01 ). Conclusions:Sperms of different parameters used in ICSI treatment can not affect the fertilization rate. Severe teratozoospermia can reduce the implantation and pregnancy rates after ICSI treatment.
