普伐他汀对不稳定型心绞痛患者血清白介素-6、10和基质金属蛋白酶-9水平的影响及疗效观察

摘　　要：目的探讨普伐他汀对不稳定型心绞痛患者IL-6、IL-10和MMP-9水平的影响及疗效观察。方法选择不稳定型心绞痛患者80例,随机分为观察组及对照组,两组患者均予以阿司匹林、肝素、硝酸酯类、β-受体阻滞剂及血管紧张素转换酶抑制剂等治疗,观察组患者加用普伐他汀片20 mg,1次/d,连用8周。结果治疗8周后,观察组患者TC、TG和LDL-C水平较前明显下降、HDL-C水平较前明显上升(P<0.05)。而对照组治疗前后血脂水平无明显变化(P>0.05);同时两组患者血清IL-6和MMP-9水平较前明显下降,IL-10水平较前明显上升(P<0.05或P<0.01),且观察组下降或上升幅度较对照组更明显(P<0.05);观察组患者的临床总有效率明显高于对照组(χ2=4.22,P<0.05)。结论普伐他汀治疗不稳定型心绞痛临床效果较好,能明显降低血脂水平,调节血清炎症介质水平,减轻斑块炎症反应,增强冠脉粥样硬化稳定性。Objective To discuss the influence and curative effect of pravastatin on serum lnterleukin-6 and l 0 （IL-6 and 10） and matrix metalloproteinase-9 （MMP-9） of unstable angina peetoris patients. Methods Eighty cases of un- stable angina peetoris patients were selected and divided into observation group and control group at random. The pa- tients in two groups were given aspirin, heparin, nitrate esters, 13-Receptor blockers, angiotensin converting enzyme inhibitor and etc. The patients in observation group were additionally given pravastatin tables（20 rag） once daily for 8 weeks. Results After 8 weeks＇ medical treatment,the TC,TG and LDL-C levels of patients in observation group de- clined obviously than before,while the HDL-C level rose obviously than before （P〈0.05）.The blood fat of patients in control group appeared no obvious change before and after medical treatment （P〉0.05）.The serum IL-6 and MMP-9 levels of patients in two groups declined obviously than before,while IL-10 levels rose obviously than before （P〈0.05 or P〈0.01 ）, and the declining or rising rate in observation group was much higher than that in control group（P〈0.05）. Tile total clinical efficiency of patients in observation group was much higher than that in control group （X2=4.22,P〈 0.05）. Conclusion Pravastatin has reliable clinical curative effect on unstable angina peetoris patients,which can ob- viously reduce the blood fat levels, adjust the express level of the serum inflammatory mediators,reduce the inflamma- tory reaction,and enhance the stability of coronary atherosclerosis.
