超声引导下锁骨下血管旁臂丛阻滞与超声引导下肌间沟臂丛神经阻滞疗效对比

摘　　要：目的对比研究超声引导下锁骨下血管旁臂丛阻滞和超声引导下肌间沟臂丛阻滞两种不同方法在上肢手术中的麻醉效果及安全性，评估超声引导下锁骨下动脉旁臂丛阻滞方法的临床价值，为临床臂丛阻滞提供安全、快速、简便、阻滞更完全的人路方法。方法我院120例ASA分级I～Ⅱ级的上肢手术患者为受试对象，采取随机数字表法分为实验组与对照组；实验组行超声引导下锁骨下血管旁臂丛阻滞；对照组行超声引导下肌间沟臂丛阻滞。对比两组麻醉操作时间、麻醉起效时间、麻醉成功率、麻醉效果、麻醉并发症发生率。结果实验组麻醉操作时间明显短于对照组（P〈0．05）；两组麻醉起效时间、成功率、效果、并发症发生率比较无统计学意义（P〉0．05）。结论超声引导下行锁骨下动脉旁臂丛阻滞方法，因为有锁骨下动脉的指引使其定位迅速、准确，是一种安全、快速、简便、阻滞更完全的入路方法，值得在临床应用中大力推广。Objective To conduct comparative research on anesthetic effect and safety of two different methods, namely ultrasound guided subclavian perivascular brachial plexus block and ultrasound-guided interscalene braehial plexus block on upper extremity surgery, to assess clinical value of lateral brachial plexus of ultrasound-guided sub- clavian artery block method, and to provide a safe, fast and simple and completer block of entry portal method in clinical braehial plexus block. Methods 120 cases of patients with I - II grade upper limb surgery of ASA grading in People＇s Hospital of Ruian City were selected , and they were divided into experimental group and control group by random number table method; The experimental group underwent ultrasound guided subclavian perivascular brachial plexus block; and the control group underwent ultrasound guided interscalene braehial plexus block. The operation time, onset time, success rate, effect and complications incidence of the anesthesia in two groups were compared. Results The anesthesia operation time in the experimental group was significantly shorter than that in the control group （P 〈0.05）;the comparison between the onset time, success rate, effect and complication rate of the anesthesia in two groups was not statistically significant（P〉0.05）. Conclusion Conduct the subclavian artery brachial plexus block, for the pointing of the subclavian artery made it be quick and accurate positioning, it is a safe, fast, simple and com- pleter entry portal method, and it is worth promoting in clinical application.
