脑钠肽在老年心力衰竭诊断及预后评估价值分析

摘　　要：目的研究BNP在老年心力衰竭患者中的血清变化情况,探讨BNP在老年心衰的诊断及评估中的临床应用价值。方法比较不同NYHA心功能分级和老年心衰患者血清BNP、cTn1、MYO实验室指标;研究BNP与LVEDD、LVEF相关性。结果 BNP除心衰I级组与对照组无统计学差异(P>0.05)外,其他任一两组均有统计学差异(P<0.05);cTn1、MYO心衰Ⅲ级组、IV级组与其他各组比较均有统计学差异(P<0.05);179例老年心力衰竭患者BNP与LVEDD、LVEF经Spearman相关性分析,LVEDD与BNP呈正相关(r=0.714,P<0.05),LVEF与BNP呈负相关(r=-0.639,P<0.05)。结论 BNP在心力衰竭的早期诊断上有重要的临床应用价值,值得临床应用推广。Objective To study the changes of serum BNP in elderly patients with heart failure, and to evaluate BNP in diagnosis and assessment of the value in the clinical application of senile heart failure. Methods Compared differ- ent NYHA heart function classification senile heart failure patients serum BNP, cTnl, MYO laboratory indicators;and Studied the BNP and LVEDD, LVEF correlation. Results BNP in heart failure I group and the control group showed no significant difference （P 〉 0.05）, one of the other two groups were statistically different（P 〈 0.05）; cTnl, MYO, IV 11/ group, heart failure group compared with other groups was statistically significant （P 〈 0.05）; 179 cases of BNP and LVEDD in elderly patients with heart failure, LVEF by the Spearman correlation analysis, LVEDD was positively correlated with BNP （r = 0.714, P 〈 0.05）, LVEF was negatively correlated with BNP （r = -0.639, P 〈 0.05）. Con- clusion BNP has important clinical value in the early diagnosis of heart failure, worthy of clinical application.
