摘 要:目的探讨E-test法、纸片扩散法和界值法检测幽门螺杆菌(Helicobacter pylori,Hp)药物敏感性的优缺点及一致性。方法采用界值法、纸片扩散法和E-test法对20株Hp临床菌株进行甲硝唑、克拉霉素、四环素和阿莫西林的药物敏感性检测。结果界值法中,各临床菌株在对照平板上均良好生长,根据在界值平板上生长情况,获得20株Hp对甲硝唑、克拉霉素、四环素和阿莫西林的药物敏感性结果;E-test法中,20株Hp有12株检测到MIC(minimum inhibitory concentration,最低抑菌浓度),8株因抑菌圈不规则,未获得MIC值;纸片法检测中,10株Hp观察到规则的抑菌圈并测量出抑菌圈直径,另10株抑菌圈不规则或不明显,未能测量到抑菌圈直径。三种方法均有检测结果的10株Hp中9株对克拉霉素、阿莫西林、四环素和甲硝唑的药物敏感性结果一致;1株对甲硝唑的敏感性,以E-test法检测为敏感,以琼脂稀释法和纸片扩散法检测结果却为耐药。结论三种药物敏感性检测方法中,界值法较E-test法和纸片法稳定;三种方法检测克拉霉素、阿莫西林和四环素的药物敏感性结果一致,但甲硝唑的药物敏感性结果不一致。Objective To investigate merits, demerits and result consistency of E-test, Kirby-Bauer, and Break point method on detecting the antimicrobial susceptibility of Hp(Helicobacter pylori). Methods Antimicrobial susceptibility to metronidazole, clarithromycin, amoxiciUin, and tetracycline of 20 Hp isolates was detected by E-test, Kirby-Bauer, and Break point methods. Results Antimicrobial susceptibility of 20 Hp strains could be detected by Break point method. MIC valued of 12 in 20 strains could be judged by E-test.Resu Its of other 8 strains could not be judged because of irregular inhibitory zone. Diameters of inhibitory zone of 10 strains could be detected by Kirby-Bauer method, but antimicrobial susceptibility of other 10 strains could not be judged because of irregular or indistinct zone. The antimicrobial susceptibility of 9 Hp strains was consistent among clarithromycin, amoxicillin, tetracycline, and metronidazole. 1 Hp strains was susceptible to metronidazole by E-test, but was judged to be resistant by Kirby-Bauer and Break point methods. Conclusions Break point method was more stable than E-test and Kirby-Baner method. Antimicrobial susceptibility results were consistent among clarithromycin, amoxicillin and tetracycline by three methods of susceptibility test except metronidazole.
