320例甲亢^(131)I治疗患者的随访分析

摘　　要：目的评价131I治疗甲状腺功能亢进(甲亢)的效果并了解患者对治疗后疾病转归的认知情况。方法采用电话联系患者或其家属的方式对320例131I治疗>1年但<2年的甲亢患者进行随访,对131I治疗的效果进行评价,并了解1次治疗未愈的患者对2次治疗的知晓情况,以及治疗后出现甲减的患者对甲减的症状和体征的认识与治疗情况。结果 320例患者经1次131I治疗后,治愈79例(占26.69%),好转71例(占22.19%),复发4例(占1.25%),无效5例(1.56%),甲减161例(占50.31%);在所有经1次治疗未愈的90例患者中,18例不知道可以进行第2次131I治疗;38例未按医嘱定期门诊随访的甲减患者中,7例对甲减控制不满意,另有8例患者有明显的甲减症状但未及时就诊。结论 131I治疗甲亢疗效确切、安全、经济,随访对于1次治疗未愈的患者和治疗后甲减患者的进一步诊治具有重要意义。Objective To evaluate the effect of hyperthyroidism therapy with 131I and learn patients＇ acknowledgement of disease prognosis. Methods Follow-up was performed in 320 cases with hyperthyroidism after single dose of 131I over one year but less than two years.Therapeutic effect was evaluated.Meanwhile,the information of patient＇s acknowl- edgement about the second treatment and how patients acquainted themselves with symptoms and signs of hypothy- roidism as well as its treatment was acquired. Results After the once of therapy with 1311,79 patients （accounted for 26.69%） were cured,improvement in 71 patients （accounted for 22.19%）,recurrent hyperthyroidism in 4 patients（ac- counted for 1.25%）,and treatment resistance in 5 patients （accounted for 1.56%）,hypothyroidism in 161 patients（ac- counted for 50.31%）.Among the 90 patients who had failed to be cured after a single treatment with 1311,18 patients knew little about second treatment with J31I.In 38 patients who had developed hypothyroidism without regular follow- up,7 patients were not treated well and 8 patients did not see their doctor in time even after seriously hypothyroidism had occurred. Conclusion Hyperthyroidism therapy with 131I is highly effective,safe and economic.Follow-up plays a very important role for patients with a minor improvement or hypothyroidism in further treatment guidance.
