摘  要:目的探讨高频超声评价慢性肾功能衰竭行血液透析(HD)患者动脉粥样硬化的价值,并分析HD患者动脉粥样硬化进展的相关危险因素。方法 58例慢性肾功能衰竭患者根据透析时间分为长期透析组(透析龄≥2年)26例及短期透析组(透析龄<2年)32例,以本院健康体检人员40例为对照组,对比分析2个病例组和对照组的颈动脉内中膜厚度(CIMT)及斑块,以及临床及实验室检查结果。结果长期透析患者的CIMT显著高于短期透析患者,CIMT的增加与血清C反应蛋白、白蛋白及钙磷水平相关。结论高频超声能够有效评价HD患者动脉粥样硬化的进展情况。Objective To investigate the value of higb-frequency ultrasonography in the evaluation of the progress of atherosclerosis in patients with chronic renal failure undergoing hemodialysis ( HD), and to analyze possible risk factors associated with the progress of atherosclerosis in HD patients. Methods In accordance with the duration of dialysis, 58 patients with chronic renal failure were divided into two groups: the long duration group (HD duration≥2 years, n =26) and the short duration group (HD duration 〈2 years, n = 32) , and forty medical personnel of the hospital receiving normal health check-ups were used as the control group. Carotid artery intima- media thickness (CIMT), plaque, clinical and laboratory test data were compared and analyzed between the 2 study groups and the control group. Results CIMT in the patients of the long duration dialysis group was significantly thicker than that in the patients of the short duration dialysis group. The increase in CIMT was correlated with the levels of C-reaction protein (CRP), serum albumin, calcium and phos-phorus. Conclusion Carotid artery seemed to be an ideal superficial window, which could reflect the seriousness of atherosclerosis in patients with HD. High-frequency uhrasonography could effectively evaluate the progress of atheroselerosis in HD patients.
