恩度联合顺铂治疗非小细胞肺癌恶性胸腔积液近远期疗效

摘　　要：目的探讨恩度联合顺铂治疗非小细胞肺癌恶性胸腔积液近期与远期疗效。方法临床入选42例非小细胞肺癌恶性胸腔积液患者,按照随机数字方法分成两组,每组21例。对照组采用顺铂40 mg胸腔内注入,每周1次,连续3周;研究组患者胸腔内注入恩度30 mg,每周2次,同时在胸腔内注入顺铂40 mg,每周1次,连续3周。观察两组患者的近期与远期疗效以及毒副作用。结果研究组胸水改善总有效率、生活质量改善率、生存时间、1年生存率、2年生存率较对照组明显增高(P<0.05)。两组患者在胃肠道反应、发热、乏力、白血病减少、血小板减少、肝肾功能损害、心电图改变等方面的毒副反应均无统计学差异(P>0.05)。结论恩度联合顺铂治疗非小细胞肺癌恶性胸腔积液能够更好地减少胸腔积液,改善生活质量,延长生存期。Objective To investigate the short- term and long- term efficacy of endostar combined with cis- Diaminedichloroplatinum （CDDP） in treating malignant pleural effusion of non- small cell lung cancer. Methods Forty- two patients with malignant pleural ef- fusion of non- small cell lung cancer were selected and randomly divided into 2 groups, twenty- one cases in each group. The control group was given CDDP 40 mg by the pleural cavity injection, once per week for continuous 3 weeks; the study group was given endostar 30mg by the pleural cavity injection, twice per week, and simultaneously pleural cavity injection of CDDP 40 mg, once per week for continuous 3 weeks. The short- term and long- term efficacy, toxic reactions and side effects were observed in the two groups. Results The total effective rate of the pleural effusion improvement, improvement rate of life quality, survival time, 1- year survival rate and 2- year survival rate in the study group were significantly increased compared with the control group（ P 〈 0. 05）. However, the toxic reactions and the side effects of gastrointestinal reactions, fever, fatigue, leukopenia, thrombocytopenia, damage of liver and renal function and EKG change had no statstical differences between the two groups（ P 〉 0. 05）. Conclusion Endostar combined with CDI）P in treat- ing malignant pleural effusion of non- small cell lung cancer can be better to reduce the pleural effusion, improve the quality of life and prolong the survival time.
