摘　　要：目的观察以抗疟药羟氯喹(HCQ)为主的联合化学治疗方案治疗糜烂型口腔黏膜扁平苔藓(OLP)的临床疗效。方法选取2001年1月至2011年2月间收治糜烂型OLP患者580例,从临床应用HCQ治疗效果的角度进行计划性分析和研究,着重观察疗效和毒副反应。结果按近期疗效标准分为显效、有效和无效3组,结果显示观察组467例中显效率为34.04%,有效率为55.03%,总有效率达89.07%,远高于对照组的37.16%;毒副反应发生率为20.34%,主要集中于皮肤、黏膜和神经系统反应方面的诸如色素沉着、脱屑、头晕头痛、失眠等轻型症状,其中最严重的为视网膜病变,仅发生1例,停药后病情稳定。结论针对OLP的治疗,以HCQ为主的联合化学治疗方案可消除糜烂和降低恶变危险度,产生的毒副反应可控,不会影响其临床应用。Objective To investigate the clinical efficacy of antimalarial drugs hydroxidechloroquine（HCQ）-based combination therapy in treating erosive type oral lichen planus（OLP）. Methods Five hundred and eighty patients with OLP admitted to our hospital were ana- lyzed and studied from the perspective of clinical efficacy of HCQ, to observe the effect and adverse reactions especially. Results The patients were divided into significantly effective, effective and invalid effective group according to standard of recent treatment effect. In the 467 cases of observation group, the significantly effective rate and effective rate were 34. 04% and 55.03% respectively, the total effective rate was 89.07%, which was obviously higher than that in control group（37. 16% ）. The adverse reaction rate was 20. 34%, which mainly focus on the skin, mucous membrane and nervous system responses such as pigmentation, desquamation, dizziness, headache, insomnia and other light symptoms, and the most serious adverse reaction is retinopathy, only lcase, who was in a stable condition after drug withdrawal. Conclusion For the treatment of erosive OLP, HCQ- based combination therapy in treating erosive OLP is expected to eliminate erosion and reduce the risk of progression, and the adverse reaction can be control Without affecting its clinical application.
