摘 要:目的探讨人类微小病毒B19感染与类风湿关节炎之间的相关性,为临床预防治疗提供依据。方法选取2009年5月-2011年12月收治类风湿关节炎患者60例设为观察组,以同期体检健康者80名设为对照组;比较两组患者人类微小病毒B19感染阳性率,B19感染阳性患者与阴性患者关节肿胀、压痛、类风湿因子阳性率,血沉、血红蛋白、C-反应蛋白及IgG水平等。结果观察组患者人类微小病毒B19感染阳性率41.7%明显高于对照组3.75%,组间比较差异有统计学意义(P<0.05);人类微小病毒B19感染阳性与阴性患者关节肿胀、压痛例数组间比较差异无统计学意义;人类微小病毒B19感染阳性患者类风湿因子阳性率64.3%、血沉(93.8±7.4)mm/h、C-反应蛋白(65.4±4.7)mg/ml及IgG水平(19.6±2.4)均明显高于感染阴性患者,但血红蛋白水平(45.8±6.9)g/L明显低于感染阴性患者,组间比较差异有统计学意义(P<0.05)。结论人类微小病毒B19感染是诱发类风湿关节炎发生的重要因素,并参与到疾病进展过程中,介导机体免疫应答反应及炎性介质释放。OBJECTIVE To investigate the relationship between the bacterial human parvovirus infections and the rheumatoid arthritis so as to provide basis for the clinical treatment. METHODS A total of 60 patients with rheumatoid arthritis for the observation group and 80 healthy people for the control group were chosen in the period from May 2009 to Dec 2012 ; the positive rate of human parvovirus B19 infections between the control group and the observation group, the joint swelling and tenderness occurrence, the positive rate of rheumatoid factor, erythrocyte sedimentation rate, and the level of hemoglobin, C-reaction protein or IgG were compared between the patients with positive and negative B19 infections. RESULTS The positive rate of human parvovirus B19 infection of the observation group was 41.7%, significantly higher than 3.75 % of the control group, the difference between the two groups was statistically significant (P 〈0. 05) % the difference in the number of the cases with ioint swelling or the cases with tenderness between the patients with positive and negative B19 infections was not statistically significant. The positive rate of rheumatoid factor of the patients with positive human parvovirus B19 infection was 64.3%, the erythrocyte sedimentation (93.8 ±7.4)mm/h, the level of C-reaction protein (65.4± 4.7)mg/ml, and the level of lgG (19.6±2.4) , which were significantly higher than those of the patients with negative infections, however, the level of hemoglobin of the observation group was (45.8 ±6.9)g/L, significantly lower than that of the control group, the difference was statistically significant(P%0.05). CONCLUSION The human parvovirus B 19 infection is an important factor to induce the rheumatoid arthritis and may involve in the progress of disease through the mediated immune responses and the release of inflammatory mediators.
