支原体肺炎患儿合并心肌损伤心肌酶谱的变化分析

摘　　要：目的探讨支原体肺炎合并心肌损伤患儿心电图特征和心肌酶谱的变化,为临床支原体肺炎合并心肌损伤提供一定的诊断依据。方法选取2010年6月-2012年6月收治的537例支原体肺炎患儿和健康儿童59名作为研究对象,支原体肺炎患儿按照是否合并心肌损伤分为心肌损伤组和非心肌损伤组,分别分析其心电图和心肌酶谱的变化。结果 537例支原体肺炎患儿中有54例发生支原体肺炎合并心肌损伤,其发生率为10.05%,患儿均表现为心电图异常,其中窦性心动过速占22.22%,窦性心动过缓占27.78%,ST-T改变占24.07%,房性心律失常占5.56%,室性心律失常占9.26%,传导阻滞占11.11%;心肌损伤组患儿心肌酶谱各指标较非心肌损伤和正常组明显增高,而非心肌损伤组患儿乳酸脱氢酶、肌酸激酶、a-羟丁酸脱氢酶、同工酶等指标水平较正常组高,差异均有统计学意义(P<0.05)。结论支原体肺炎合并心肌损伤患儿心电图和心肌酶谱均表现异常,可作为临床诊断方法。OBJECTIVE To explore changes of electrocardiogram and myocardial enzymogram in children with mycoplasma pneumonia complicated with cardiac damage and provide a theoretical basis for the clinical diagnosis of the disease. METHODS Totally 534 children with M. pneumonia and 59 health children were selected in the hospital from Jun 2010 to Jun 2012. 534 children with M. pneumonia were divided into cardiac damage group and cardiac normal group. Electrocardiogram and myocardial zymogram were analyzed in the three groups. RESULTS The incidence rate of M. pneumonia complicated with cardiac damage was 10. 050//00. All children in cardiac damage group exhibited abnormal electrocardiogram, such as sinus heartbeat tachycardia （22. 22%）, sinus heartbeat brad ycardia （27.78 % ）, ST-T changing （24.07 % ）, atrial arrhythmia （5.56% ）, ventrieular arrhythmia（9.26 % ）, conduction block （11.11 %）, Myocardial zymogram level （LDL, CK, a-HBDH, CK-MB） in the cardiac damage group were significantly higher than those in the cardiac normal group and the health group （P〈0.05）. LDL, CK, a-HBI）H and CK-MB level in the cardiac normal group were significantly enhanced compared with those in the health group （P〈 0. 05）. CONCLUSION ECG and myocardial enzymes in children with M. pneumonia complicated with cardiac damage are abnormal, which can be used for the chinical diagnosis.
