眼科手术室相关人员锐器伤调查分析及预防控制

摘　　要：目的调查分析眼科手术室人员锐器伤发生的原因,探讨实施有效的防护措施,减少锐器伤的发生。方法采用问卷调查方式,于2011年1-12月对手术室护士、手术医师、眼科实习医师、器械清洗人员及护工分别进行锐器伤的回顾性调查,对锐器伤发生原因、锐器种类及操作环节等进行分析;针对调查分析结果采取相应的防护措施,包括采用防护性的显微手术刀,规范锐器的使用、回收及清洗流程,对不同岗位人员进行针对性的培训等。结果眼科手术室人员锐器伤发生率前3位依次为器械清洗人员100.0%、工作年限<3年的手术室护士46.2%、实习医师28.6%;致伤物依次为显微手术刀、头端尖锐的显微器械、注射器针头及缝针。结论眼科手术室因使用器械精细、手术周转快、医护人员接触尖锐器械多,是锐器伤发生的高危科室,而眼科器械清洗人员是高危人群,通过针对性地预防干预,可以有效地预防锐器伤的发生。OBJECTIVE To investigate and analyze the causes of sharp instrument injuries in operating room of ophthalmology depatrtment as well as implement the protective measures to reduce the incidence of sharp instrument injuries. METHODS A retrospective investigation was conducted on the causes of sharp instrument injuries in nurses, surgeons, graduate students, cleaning auxiliary nurses and nursing workers by questionnaire from Jan. 2011 to Dec. 2011. The analysis was conducted regarding to causes for injury, types of sharp instruments and operation steps. According to the investigation results, corresponding preventive measures such as using protective microsurgical instruments, making normative processes of microsurgical instrument cleaning and conducting pertinent trainings among clinical personnel were adopted. RESULTS The first three highest incidences of sharp instrument injuries were observed among the following different clinical personnel： cleaning staff （100.0%）, nurses with less three years of woring experience （46.2%）, and intern doctors （28.6%）. The sharp microsurgical instruments with high hurting rate were in turn as follows： the microsurgery knife, sharp mierosurgery instrument, syringe needle, and sewing needle. CONCLUSION An ophthalmology operation room is a high risk department because of fine microsurgery instruments, fast working efficiency and more chances of exposure to sharp instruments. The cleaning staff have a high rate of sharp instrument injuries. Incidence of sharp instrument injuries can be effectively reduced through the implementation of specific preventive interventions.
